DEERS Enrollment

Enrollment:

• New Spouse

o  Sponsor - Military Member or Retiree must be present or customer must present a valid Power of Attorney (POA), special or general.
o  Marriage Certificate
o  Social Security Card
o  Sponsor’s Divorce Decree (If Applicable)
o  Two (2) Valid Forms of Identification
o  Birth certificate
o Primary ID must be government issued photo ID.

• Newborns

o  Sponsor - Military Member or Retiree must be present or customer must present a valid Power of Attorney (POA), special or general.
o  Proof of Birth (From Hospital), Birth Certificate, OR another official Live Birth Document.

o *Not required immediately* Social Security Card (Due within 90 days of birth).

NOTE: If the spouse is entering the child or children into the system, he/she will need to be in DEERS under the sponsor and have a general power of attorney.

• Stepchildren

o  Sponsor - Military Member or Retiree must be present or customer must present a valid Power of Attorney (POA), special or general.
o  Marriage Certificate
o  Birth Certificate
o  Social Security Card
o  Two (2) Valid Forms of Identification (18 yrs. & older)

NOTE: If the spouse is entering the child or children into the system, he/she will need to be in the DEERS under the sponsor and have a general power of attorney.

• Wards & Foster Children

o Sponsor - Military Member or Retiree must be present or customer must present a valid Power of Attorney (POA), special or general.
o  Court Documentation (Legal and Physical Custody)
o  Birth Certificate
o  Social Security Card
o  Two (2) Valid Forms of Identification (18 years & older)

NOTE: If the spouse is entering the child or children into the system, he/she will need to be in the DEERS under the sponsor and have a general power of attorney.

•  Renewal of CACs:

o  Geneva Convention Identification Card & Civilian CAC

·         Update CAC 30 days prior to expiration

·         30 days prior to expiration if CAC is still working.

·         Must bring current CAC at time of renewal

o  Contractor’s CAC & Other Federal Affiliate’s CAC Updated information in CVS with a new expiration date.

·         Must bring current CAC at time of renewal

Two (2) Valid Forms of Identification

NOTE: Contact POC to ensure that information has been updated in CVS.


Renewal of Dependent ID Cards:

An online, fill DD 1172-2 form may be accessed by clicking HERE.

• RAPIDS Self Service

• Spouse

o  Sponsor - Military member or Retiree must be present; OR customer must present a Special or     General Power of Attorney; OR a DD Form 1172-2 (see note)
o  Two (2) Valid Forms of Identification
o Primary ID, must be government issued photo ID

NOTE: The DD Form 1172-2 should be generated from a DEERS office or filled out, signed, dated and notarized by the sponsor.

• Children (10yrs. – 20yrs.)

o  Sponsor - Military member or Retiree must be present; OR customer must present a Special or General Power of Attorney; OR a DD Form 1172-2 (see note)

NOTE: The DD Form 1172-2 should be generated from a DEERS office or filled out, signed, dated and notarized by the sponsor.



• Sponsor - Military member or Retiree must be present; OR customer must present a Special or General Power of Attorney; OR a DD Form 1172-2 (see note)

o  Can come 90 days prior to 21st Birthday with the following.
o  School Letter (The letter should state that the individual is a full time student and his/her expected      graduation date.)
o Two (2) Valid Forms of Identification
o Primary ID, must be government issued photo ID

NOTE: The DD Form 1172-2 should be generated from a DEERS office or filled out, signed, dated and notarized by the sponsor. The DD Form 1172-2 must state the sponsor is providing over 50% of the annual support for the dependent child/children.

Replacement Identification Card:

• Lost or Stolen cards

o  Sponsor - Military member or Retiree must be present; OR customer must present a Special or     General Power of Attorney; OR a DD Form 1172-2 (see note)
o  Two (2) Valid Forms of Identification
o Primary ID, must be government issued photo ID

NOTE: The DD Form 1172-2 should be generated from a DEERS office (which can be faxed from ID office to ID office) or filled out, signed, dated and notarized by the sponsor.

• Promotions

o  Current Orders
o  Two (2) Valid Forms of Identification
o  Must turn in current CAC at time of replacement

Geneva Convention (GC):

• Senior Executive Service (SES) member

o Travel orders or memorandum from organization
o Two (2) Valid Forms of Identification
o Primary ID must be government issued photo ID

• Civilians

o Travel orders or memorandum from organization
o Two (2) Valid Forms of Identification
o Primary ID must be government issued photo ID

• Contractors

o Entered into DEERS through TASS
o LOA with travel dates / LOA must be signed within 30 days of card issuance
o Two (2) Valid Forms of Identification
o Primary ID must be government issued photo ID

NOTE: Contractors are eligible for one of the following forms of CACs dependent on deployed status:
   * GC CAC – LOA stating “Mission Essential”
   * DoD IP CAC – LOA with travel dates of 365 days or more
   * DoD ID CAC – LOA with travel dates for less than 365 days

• Other Federal Affiliates {Department of State (DOS) and Federal Bureau of Investigation (FBI)}

 

o Entered into DEERS through TASS
o LOA
o Two (2) Valid Forms of Identification

Other Transactions:

     o Divorced Sponsor - Army Personnel ONLY
     o Statement of Service or DD 214 or Dates of inclusive service from the servicing personnel
     o Final Divorce Decree


•   Divorcees (Dependents) - Army Personnel ONLY o  Social Security Card

o  Current Address
o  Driver’s License
o  Marriage Certificate - Sworn Statement. Statement must say that you have not remarried and     that you do not have any other health insurance and must be notarized
o  Sponsor’s DD 214 / Statement of Service
o  Final Divorce Decree
o  Two (2) Valid Forms of Identification

• Geneva Convention Identification Card (Military IDs)

o  Entered into DEERS from military personnel
o  Two (2) Valid Forms of Identification
o  1st ferm of ID must be government issues photo ID

• Civilian CAC

o  Entered into DEERS through Civilian Personnel {1 day to 2 weeks}
o  Birth Certificate
o  Social Security Card
o Government issued photo ID

•  Contractor’s CAC & Other Federal Affiliate CAC

o  Entered into DEERS through the Trusted Associate Sponsorship System (TASS)
o  NOTE: Contact POC to ensure that information has been entered into the TASS.
o  Two (2) Valid Forms of Identification
o 1st ID must be government issued photo ID

• Disabled Veteran (DAV)

o  100% Disability Letter
o  DD 214 (Honorable Discharge & Copy #4)
o  Birth Certificate (Self & Family Members)
o  Marriage Certificate (If Applicable)
o  Social Security Number (All Family Members)
o  Two (2) Valid Forms of Identification (21 yrs. & older)
o 1st ID must be government issued photo ID

• Foreign Military

o  Birth Certificate (Self & Family Members)
o  Marriage Certificate (If Applicable)
o  DD Form 1172-2 signed and verified by the embassy
o  Two (2) Valid Forms of Identification (21 yrs. & older)
o 1st ID must be government issued photo ID

Note: For initial or renewal cards for dependents, please go to the DEERS.

• Retirees

o  Retirement Orders / DD 214
o  Two (2) Valid Forms of Identification

• Renewals for DOD Beneficiary (DB)/Un-remarried Widow (URW)

o  Two (2) Valid Forms of Identification

• Initial issue of URW (Death Certificate & a forms of ID)

• Temporary Incapacitated

o  DD Form 1172-2 (Certified by DFAS)
o  DFAS-IN Form 27-88
o  Two (2) Valid Forms of Identification
o  Social Security Card
o  NOTE: The child or parent dependency packet will be filled out by the sponsor {every four (4) years} and sent to DFAS. DFAS will return the DD Form 1172-2 with a stamp approving the     dependent. The DD Form 1172-2 should be brought to the DEERS office in order to issue the ID     card.

•  Permanent Incapacitated

o Two (2) Valid Forms of Identification
