CENTRAL CLEARANCE

Effective July 7, 2014, Directive Type - Memorandum (DTM) 14-006: Separation History and Physical Examination (SHPE)

All Regular Army and Reserve Component Soldiers separating from active duty after serving for 180 days or more, or over 30 days in support of a contingency operation, will need to complete a Separation History and Physical Examination.

The SHPE will include a review of a Soldier's medical history, documented on a DD Form 2807-1 (Report of Medical History), and a physical examination, documented on a DD Form 2808 (Report of Medical Examination).

The SHPE must be completed and validated as current within 30 days before the date of spearation, consistent with Army separation procedures. If you did not in-process with JBLM, then you do not need to out-process with JBLM.

Leave forms are generated through your unit S1. Copies of your orders can be acquired through the following:

  • For PCS ORDERS, contact your servicing S-1.
  • For ETS, RETIREMENT or SEPARATION ORDERS, contact the Transition Center.
