Decision Matrix

The Battle Drill provides step-by-step guidance on how to navigate situations where a service member is distressed from environmental stressors or suicidal ideation.
- Step 1: Witness identifies a stressed service member who exhibits warning signs of suicidal behavior.
- Step 2: Witness exercises knowledge from ACE Intervention training and ACE card to determine if stressed service member has had thoughts of suicide. The witness will Ask, Care, and Escort as trained.
- Step 3: Thoughts of suicide by a stressed service member requires Company / Troop / Battery command leadership intervention. A stressed service member with thoughts of suicide will be safeguarded and cared for by the witness until they are safely escorted to their company leadership.
- Service members acting out preparation of suicide or in the act of suicide requires immediate medical attention and 911 should be called immediately.
- Service members with suicidal thoughts who are unable to get escorted to their company leadership will be escorted to a chaplain, Behavioral Health, or ER.
- Step 4: Witness does a proper handoff, sharing all precluding information, to ensure that care continues properly.
- Step 5: Command leadership CARES for stressed service member by engaging with empathetic communication.
- Ensure the stressed service member understands that they are not a burden and their leadership is committed to assisting them with overcoming or coping with their environmental stressors.
- Step 6: Command leadership builds rapport and uses open-ended fact-finding questions to determine what environmental factors are impacting the stressed service member.
- It is common for stressed service members to have more than one environmental stressor impacting them.
- Step 7: Command leadership uses the Courage CARES Resource Guide to develop a plan that will help address applicable environmental factors impacting the stressed service member.
- Company / Troop / Battery Commander is required to approve resource plan on a Memorandum for Record (MFR).
- Step 8: Command leadership will incorporate the use of the Decision Matrix to assist with identifying risk level potentially requiring Behavioral Health assistance or intervention.
- Command leadership should seek guidance from their unit Behavioral Health Officer or chaplain as needed.
- Service members will never be denied any requests to see Behavioral Health.
- Step 9: Witness or trusted leader is assigned as a Courage CARES mentor to ensure the stressed service member has assistance with the outlined resource plan developed for them.
- Team leaders or squad leaders are good options to assist service members.
- Step 10: Command leadership continues to follow up on the stressed service member to ensure that the resource plan established is alleviating pressure from environmental stressors.
- Command leadership will keep higher command informed as needed.

- Step 1: To access CARES Power Application, click here.
- May be prompted for App Permissions, click “Allow"
- Step 2: I Corps C.A.R.E.S Form will display. Populate all fields in the forms with SIR/CCIR related information as you would for the SIR/CCIR document.
- ALL FIELD MUST BE FILLED OUT. Use accurate, detailed, or best judgement when populating information. “Unknown” and “NA” are allowed but should be used sparingly.
- The form is large so you may have to scroll down to see the rest of the form. Summary Fields are for narratives or detailed plans of action.
- Note: Utilize CARES website to locate and identify resources that will be used for the subjects CARES Plan. Sponsor, Command, and Point of Contact must be accurate with phone number.
- Step 3: Once ALL fields are complete, scroll to the top of the form and click “Submit.”
- Step 4: Form will clear to verify submission. Shortly after submission (3-5 minutes) you should receive an automatically generated email with an SIR Word document attached.
- Open the document and verify that the contents match what was entered into the CARES Application. Adjust as needed then route document through your SOP directed SIR/CCIR process.
- Step 5: Once an O6 level or higher commander has approved the SIR Word document send the Word document to this email: CourageCARESReporting@army.mil

REACH is a proactive mindset. REACH aims to change the culture of help-seeking in the military by placing the focus on the individual and their own need to engage in selfcare and not suffer in silence. Available to all service members, family members, and DA civilians.
The optimal group size is 10 participants (min. 5 / max. 15). Participants should be grouped by rank to promote open sharing.
- Ask, Care, Escort - Suicide Intervention (T4T) Training
- Training Available to E-6 and above and teaches the skills to intervene in a suicide situation. Content of the training includes communication skills, risk and protective factors, stigma, warning signs, intervention skills, and the ACE method of suicide prevention.

The Family Advocacy Program - Child Abuse Prevention provides education to Soldiers and family members through training and classes with the goal of reducing and preventing child abuse.

FOCUS Resilience Training is designed to strengthen couples and families in readiness for tomorrow. FOCUS provides personalized training in the context of each family’s story and individual goals. It teaches families practical skills to help them meet the challenges of military life such as deployments, stress, injury, and other transitions.
Classes Offered:
FOCUS Resilience Training - FOCUS is a multi-session resilience training program for active-duty military families and couples. Sessions are organized around the development of a family timeline which is used to help families manage feelings, improve communication, and learn problem solving and goal setting skills.
Consultations - Consultations offer education to community providers and partners, as well as to parents and couples.
Skill Building Group - Skill Building Groups provide an introduction to the key skills taught in FOCUS resilience training.
Educational Workshops - Workshops generally cover a specific topic, such as individual or unit level stress management or developmental reactions to deployment or other transitions.
Briefs - Briefs typically include an overview of FOCUS, education about the impact of military life on couples and families and the types of resilience training strategies that can be helpful.

- Engage Training - There may have been times when you wished you’d noticed an issue sooner or stepped in or alerted someone about a concern. Even though you cared, you may not have known how. Research on pro-social behavior has shown that helping others improves our connections and trust in others. Offering someone help is a conscious process and is dependent on the ability to say “yes” at critical choice points. Engage training teaches what those choice points are, how to notice them, how to take responsibility, and how to act in critical moments to help a battle buddy before a problem escalates.
- Great Teams Workshop - Great Teams is a culture development program that brings together selected Soldiers and leaders from across the unit to engage in exercises and discussions that solidify or redefine the unit’s shared philosophy, vision, values and standards, which compose a unit’s culture. Defining an intentional culture gives the unit a foundation on which to build cohesive, well-led teams committed to the Army professional ethics.
- Squad Leader Development Course - The Army leadership requirements outlined in ADP 6-22 emphasize integrating, demonstrating and sustaining Army Values. The leadership core competencies include the ability to build trust, communicate and create a positive environment. This course develops squad leaders’ leadership skills by teaching concepts like communication, empathy and trust. Squad leaders also learn how to have tough conversations and practical strategies for leading and developing others.
- Team Building - Sometimes a team can benefit from getting to know each other, kicking off a training event with an activity that energizes the group or learning to work together to overcome challenges. R2 Performance Center trainers customize activities that help teams get to know each other better, learn about their communication and collaboration styles and have fun while learning to work together.

The JBLM Children's Museum is a pay as you will facility. At the JBLM Children's Museum we believe play is a crucial component of children’s learning. Chaplain's Corner
The JBLM Religious Support Office provides a variety of free support services offered to service members, families, civilians and retirees designed to help you develop and strengthen your most important relationships in life.
Services Offered:
- Chaplain Support: All service members stationed at JBLM, and their family members have an assigned chaplain. Please contact them first when you are in need of chaplain support. If you do not know who your chaplain is, please call the Religious Support Office at 253-967-3718
- Chapels: The JBLM RSO oversees and operates nine chapels, including three on Lewis Main, three on McChord Field and one on Lewis North.
- Worship Services: JBLM provides diverse religious services for Soldiers, Airmen, families and veterans.
- Religious Education and Auxiliary Ministries: All chapel worship services host Bible studies and/or Sunday school classes, as well as regular and special mid-week events, prayer breakfasts and luncheons.
- Chaplain Family Life Center: The pastoral counseling team at the Chaplain Family Life Center provides a variety of free services to help develop and strengthen your most important relationships in life. Counseling is free with no referral needed and is open to active duty of all branches, retirees, family members and DOD civilians.
