ACTIVE DUTY
Expired Cards: Soldier will need previously issued active duty ID card and any supporting documentation for status/rank changes.
LOST ID CARDS
Personnel must have two (2) forms of ID (one of the forms must be a picture ID (See acceptable forms of ID) and a "Declaration of Lost or Stolen Identification Cards" signed by the Fort Detrick Provost Marshall's Office.
RETIREE
Retirement orders or DD Form 214 along with two (2) forms of ID and one (1) of the forms must be a picture ID.
SPOUSE, CHILD UNDER 21 YRS OF AGE (INITIAL ENROLLMENT)
Original or Certified true copy (from place of origin), Birth Certificate or Certificate of Birth and social security card are required. The sponsor must be present to sign DD Form 1172-2 (ID Card Application) or the spouse must have the sponsor's Power of Attorney to sign for the sponsor in his/her absence or the spouse must have a DD Form 1172-2 not more than 90 days old signed by the sponsor and verified by the service member servicing ID Card Section. If the DD Form 1172-2 is not signed in the presence of the verifying official, the signature must be notarized.
CHILD BORN OUT OF WEDLOCK
If child is born out of wedlock, the sponsor(father), must provide one of the following:
1.Original court order that establishes paternity signed by a judge and the child's Birth Certificate and Social Security Card.
2.Affidavit signed by the sponsor and mother of the child. Child's Birth Certificate and Social Security Card.
3.Approved Dependency and Residency Determination worksheet. (requesting forms may be obtained in the ID Card Section). Child's Birth Certificate and Social Security Card.
GENERAL ID CARD RENEWAL
The sponsor must be present to sign DD Form 1172-2 (ID Card Application) or the spouse must have a Power of Attorney to sign for the sponsor in his/her absence or the spouse must have an original DD Form 1172-2 not more than 90 days old signed by the sponsor and verified by the service member servicing ID Card Section. If the DD Form 1172-2 is not signed in the presence of the verifying official, the sponsor must sign the form in the presence of a notary. Sponsor can also choose to go to the ID Card Office Online website and generate a DD Form 1172-2 fill it out and sign it electronically, save it to DEERS and the family member can come to the ID Card Section without the sponsor.
CHILD, AGE 21-23
The child must be enrolled in college as a Full-time student. The sponsor must present a letter from the school Registrar's office certifying that the student is enrolled full-time, to include the start date and the anticipated graduation date. The sponsor or the student can also go on the Student National Clearinghouse website: www.studentclearinghouse.org and print an enrollment verification letter. The sponsor must be present to sign DD Form 1172-2 (ID Card Application) or the student/spouse must have the sponsor's Power of Attorney to sign for the sponsor in the service member absence or the student/spouse must have a DD Form 1172-2 not more than 90 days old signed by the sponsor and the service member servicing ID Card Section. If the DD Form 1172-2 is not signed in the presence of the verifying official, the sponsor must sign the form with the school letter attached in the presence of a notary.
INCAPACITATED CHILD OVER AGE 21, PARENT/PARENT-IN-LAW DEPENDENCY (ARMY)
Army Dependency Determination can be done for a child 21 years of age who is incapacitated, parents and parent-in-laws provided the sponsor is supporting these dependents with at least 50% of their living expenses. All other branches of service must do initial requests through their respective branch of service. Requesting forms can be obtained in the ID card section. Please contact the ID card office for further instructions. This request is processed through Defense Finance and Accounting Services (DFAS) not the ID card section.
FORMER SPOUSE (ARMY)
Unmarried former spouses can continue to keep their dependent ID card privileges provided they meet the following criteria: The member served at least 20 years creditable service for retired pay, the marriage lasted at least 20 years, and the marriage and service overlapped by at least 20 years. The number of years overlaps in marriage and service determines the extent of benefits authorized. Requires original or certified true copy(from place or origin), Marriage Certificate, Divorce Decree, sponsor's DD Form 214, retirement orders or Statement of Service showing 20 years continuous service, social security card and one photo form of government or state issued ID (See acceptable forms of ID). All other branches of service must do initial requests through their respective branch of service.
100% DISABLED VETERAN
Requires a letter from the Veterans Administration stating service member is 100 percent disabled service-connected. The letter must state whether the Disabled American Veteran (DAV) is scheduled for future examinations and that the service member is entitled to Commissary and Post Exchange privileges. A DD Form 214, which states honorable discharged, social security card and one picture form of government or state issued ID (See acceptable forms of ID) are required to receive an ID card.
DOD CIVILIAN EMPLOYEE (APPROPRIATED FUND)
A record of the employee' information must be in DEERS, before the Common Access Card (CAC) can be issued. The Civilian Personnel Office creates these records when the new employees in processes. It normally takes 3-5 days before the record of the new employee appears in DEERS. Two forms of government of state issued identification and one must contain a photo (See acceptable forms of ID) and an .edu, .mil, or .gov email address. ID cards can be renewed 90 days before the card's expiration date.
DOD CIVILIAN EMPLOYEE (NONAPPROPRIATED FUND)
These employees are issued CAC based on their applications. Two forms of government or state issued IDs and one of the forms must be a picture ID (See acceptable forms of ID) and an .edu, .mil, or .gov email address are required. For renewal employees must repeat the application process not earlier than 90 days before their cards expire.
IPA/NON-DOD FEDERAL EMPLOYEE
These employees are issued CAC based on their applications. Two forms of government or state issued IDs and one of the forms must be a picture ID (See acceptable forms of ID) and military email address are required. For renewal employees must repeat the application process not earlier than 90 days before their cards expire.
RETIRED DEPARTMENT OF THE ARMY CIVILIAN
Civilian retirees must be downloaded into the DEERS system from their respective Civilian Personnel Office and (2) forms of government or state issued IDs and one of the forms must be a picture ID (See acceptable forms of ID). ID card may be renewed up to 90 days before the expiration date.
CONTRACTOR
All contractors must process through the Trusted Associate Sponsorship Systems (TASS) prior to the issuance of the CAC. The contractor's Trusted Agent (TA) or personnel staff is responsible for processing the contractor through TASS. Contractors will receive an email informing them that their applications for a CAC has been approved, then contact the ID card office to ensure the information has been updated in the DEERS system. Once verified, the contractor may go to any CAC Identification site for processing. The contractor must have two forms of government or state issued IDs and one of the forms must be a picture ID (See acceptable forms of ID).
