Routine uses: Information is used to route individuals to specialists in the most efficient way possible. Information will be seen by PPP personnel, including contractors. It will not be preserved after your session.
Disclosure: Completing the form is optional. Customers can also enter this information into a kiosk on-site. Not using this form may result in delayed service.
