MISSION
Our mission is to provide quality service to all Military Personnel, Family Members, DoD Civilian Employees, Contractors, Retired Military and Retired Civilian Employees. Services include the issuance of Identification Cards for Members of the Uniformed Services, DEERS records updates, and ID Tags.
OUR BASIC PROCEDURES
Customers are serviced on a walk-in basis. Customers must present valid identification and required documentation. Sponsors must accompany their Family members at all times. However, this facility does accept power of attorney in the absence of the Sponsor, or a DD Form 1172-2 that is generated by a DEERS/ID Cards Facility or online by visiting: https://www.dmdc.osd.mil/self_service/rapids/ and follow the steps to generate a DD Form 1172-2. You must have a CAC reader to complete the DD Form 1172-2 online. The form has to be generated within 30 days prior to your appointment.
DOCUMENTATION REQUIRED FOR ID CARD ISSUANCE
CAC: Active Duty, Reserve, Guard, Civilian, Contractor, NAF, and AAFES
- Promotion orders, E4 or above (if rank change)
- Two valid forms of ID (Acceptable Proof of Identity Documentation).
- Contractors must be verified in MPICAM (every time a card is issued)
Retired Military: DD Form 214 or retirement orders and two valid forms of ID
Individual Ready Reserve (IRR): DD Form 214 and two valid forms of ID
Dependent ID: **Sponsors must accompany their Family members, or this facility does accept power of attorney in the absence of the Sponsor, or a DD Form 1172 that is generated by a DEERS/ID Cards Facility and signed by the sponsor within the last 90 days.
- Spouse – Two valid forms of ID (Medicare Card if turning age 65).
- Un-remarried Widow – Two valid forms of ID (Medicare Card if turning age 65).
- Former Spouse – Two valid forms of ID (Medicare Card if turning age 65).
- Dependent under age 18 - No ID necessary
- Dependent over age 21 – (Student) – Two valid forms of ID, Letter from school registrar office stating full time enrollment (Verification from Clearing House is also acceptable), and the expected graduation date at accredited college.
- Dependent over age 21 – (Incapacitated Child) – Two valid forms of ID, Current Dependency Determination* Medical Sufficiency Letter
- Other Dependent – (Parent, Parent-in-Law, etc) – Two valid forms of ID, Current Dependency Determination.
*Dependency Determination must be completed every four years or status change
DAV (initial issuance): Two valid forms of ID, letter from VA stating member is 100%, and DD Form 214
WAYS TO VERIFY AND UPDATE DEERS
ONLINE: You have two options for updating personal information online:
- Visit the DEERS Web Site at: HTTPS://WWW.DMDC.OSD.MIL/APPJ/ADDRESS and follow the steps to update your address, email address, and phone numbers on line. (MUST have CAC or DFAS PIN)
- Via the Beneficiary Web Enrollment Web Site at: HTTPS://WWW.DMDC.OSD.MIL/APPJ/BWE/INDEXACTION.DO This site is linked directly to the DEERS database, so when you update your information via this portal, it not only updates DEERS, but also with your regional contractor. (MUST have a CAC or DFAS PIN)
BY PHONE: Call the Defense Manpower Data Center Support Office at 1-800-538-9552 to update your addresses, email address, and phone number. (TTY/TDD for the deaf: 1-866-363-2883)

REQUIREMENTS FOR DEERS ENROLLMENT

DEPENDENCY APPLICATION
To be considered sufficiently dependent upon the Sponsor, the claimed dependent’s qualifying income must be less than 50% of his/her expenses and the member's contribution must exceed 50% of the dependent's expenses. This can include:
- Parent, parent-in-law, step-parent, parent by adoption, or any person who stood in loco parentis to the member for at least five years prior to the member's emancipation.

Installation Passport Office
Mission Statement: Our mission is to support the United States Department of Defense (DOD) and Department of State by assisting DOD Service Members, Civilians, and Family Members in obtaining the official travel documents necessary to facilitate official government travel.
To be eligible for a No Fee (Official) Passport you must be:

Uniformed personnel on official duty that requires a special issuance passport per the Foreign Clearance Guide.

Command sponsored family members traveling overseas on orders with sponsor.

Government Civilian Employees who are assigned to Okinawa, Japan and eligible for SOFA or who have accepted a position to a country that requires a passport.
Passport Requirements
- If going on personal travel/vacation needing a Tourist Passport please call DSN: 315-652-5419 Commercial: 011-81-90-351-5419 to schedule an appointment.
- Applicant has never had a passport
- Applicant is under the age of 16
- Applicant was under the age of 16 at the time of most recent passport issuance
- The most recently issued passport was issued >15 years ago
- The passport being renewed has been lost, stolen, or is no longer in the possession of the applicant
- Applicant was at least 16 years old when most recent passport was issued
- Is in possession of that particular passport
- Passport was issued less than 15 years ago
- Name has changed by marriage or court order >1 year after most recent passport was issued and can submit original documentation to reflect legal name change
- Applications must be completed online at http://www.travel.state.gov/ using the “Form Filler”, follow their directions, then print the application. NOTE: DO NOT SIGN until prompted by a passport agent.
Exit/Reentry Stamp
To obtain the exit/reentry stamp in your passport, please bring your record brief (military) or SOFA documentation (civilian) that has your DEROS.

How do I get my orders?
Once the Reassignment Office receives your assignment, they will email your Assignment Notification to you and your S-1 with instructions on when to attend the Levy Brief. Once you have attended the levy briefing the reassignment section will cut your orders within 90 days of your DEROS.
How do I get my clearing papers?
You will receive an email from Personnel Services with the date and time that you can pick up your Clearing Papers and receive your out-processing brief. For assistance, please email at usarmy.torii.usag.list.okinawa-outprocessing@army.mil.

Casualty Assistance Center (CAC)
The mission of the Casualty Assistance Center is to provide compassionate, unwavering commitment to the Families of our fallen Soldiers, retirees and veterans. Casualty Assistance provides assistance to next-of-kin of critically injured, ill or deceased Army soldiers, retirees, and veterans to include Benefits Counseling; Military Funeral Honors; and Casualty Assistance / Notification Officer training (CAO/CNO).

Casualty Notification/Assistance Officers (CNO/CAO) Training
Casualty notification/assistance officers must attend a three-day certification course prior to performing those services. The CAC's training instructor offers this course every month.

Benefits and Entitlements
Surviving spouses of retired and active Soldiers may receive casualty assistance through three methods:
- Live assistance. The Casualty staff serves as a survivor's advocate who processes Active Duty benefit claims, coordinates services from support organizations, and answers questions related to entitlements.
- Guided assistance. The CAC can also arrange for a Casualty Assistance Officer (CAO) to visit the survivor at a location of choice on a case by case basis. The CAO will assist with completion of the forms and serve as a liaison between the survivor and the Casualty staff.
- Retiree death reporting and benefits. To report the death of an Army military retiree contact Defense Finance And Accounting (DFAS) at (800) 321-1080.
