The Fort Jackson ID Card Facility located in Room 109, Bldg. 5450 Strom Thurmond Blvd., services walk-in customers from 8-10 a.m. Monday through Friday. All appointments are scheduled from 10 a.m. to 3:20 p.m. Monday through Friday. The office closes at noon on the first and third  Thursday of each month for mandatory training. It is closed on all federal holidays.

PER THE SERVICE PROJECT OFFICER, EFFECTIVE IMMEDIATELY, THE COMMON ACCESS CARD (CAC) WILL NO LONGER BE ACCEPTED AS A FORM OF IDENTIFICATION WHEN REQUESTING A REPLACEMENT ID.  CUSTOMERS MUST HAVE TWO FORMS OF UNEXPIRED IDENTIFICATIONS.  ANY QUESTIONS REFERENCE THIS NOTIFICATION, NEEDS TO BE DIRECTED TO THE SERVICE PROJECT OFFICER AT 1-888-276-9472.

Mission:

The Directorate of Human Resources, Fort Jackson, Identification card office continues to strive and provide excellent customer service. Effective Jan. 3, 2022, walk-ins will be allocated FOR EMERGENCIES ONLY. The following constitutes an emergency: Soldiers and Department of Defense civilian/contractors with a non-functioning common access cards, customers needing a replacement CAC due to lost or stolen card, pin rest, expired card and new employment of DOD civilian/contractor employees.

Service:

* Active-Duty Soldiers can come in at 1 p.m. to update records and/or CAC (This is for Active-Duty Soldiers Only and does not include dependents)

Hours of Operation

*Walk-in hours are from 8-10 a.m. All customers ages 18 and over must have two forms of unexpired identification in hand (not including their current CAC) upon arrival.

 *We see scheduled appointments and walk-ins (Walk-ins are allocated for emergencies only) Appointments have priority.  

*We close at noon on the 1st & 3rd Thursday of each month for mandatory training. We also close for all Federal Holidays.

*Customers are authorized to renew ID Cards up to 90 days prior to the expiration date, EXCEPT CUSTOMERS TURNING AGE 65. Customers turning 65 can renew their ID cards no earlier than 10 days prior to expiration and must bring Medicare card.

 *We do accept walk-in customers until 10 minutes prior to the first appointment start time.  Walk-in Customers CAN wait for up to two to three hours. 

The following constitutes an emergency:

*Lost Retirees (to include family members) and Death Beneficiaries

*Lost Common Access Cards (CAC), expired or non-functional for Soldiers, Civilians and Contractors (Police report will start being required instead of a counseling and/or memo from the first sergeant, company commander or supervisor)

*Pin Reset (Only if Room 124 systems are down)

Effective 1 July 2025, Fort Jackson ID Cards Office will increase the number of appointments to assist more customers.  We highly encourage you to make an appointment to ensure that you receive the service that you are requesting.  YOU MUST SCHEDULE AN APPOINTMENT FOR EACH PERSON RECEIVING SERVICE. *You must arrive 15 minutes prior to your appointment. Anyone arriving 5 minutes late for an appointment will need to reschedule. .*Please note that Civilian Retiree Employees ID CARDS will no longer being issued.

 *Correcting Retiree Records in DEERS. When retirees request a correction to a record, they must start by going through the HRC Veteran Inquiry Branch which will verify and or correct their record.  * When scheduling your appointment and you do not see anything available, that does not mean that the system is not working.  That means either the date and/or time you want is unavailable.  If you see that there are no available slots for a particular date and time, you will either need to periodically check for cancellations or look for a future date and time.  Appointments are posted usually 30-60 days in advance and can be limited.  Please ensure you check early for your desired date and time.

DOW LIST OF ACCEPATABLE IDENTY DOCUMENTS:

*ID card applicants 18 years and older are required to provide two forms of unexpired identity source documents in original form upon arrival to the ID card section.

*The identity source documents must be bound to that applicant and shall not be expired.

*All source documents must be in hand; we cannot accept documents on your phone nor via email.

*All family members over the age of six months are required to have their original social security card for enrollment.

**NOTE:  IF YOU NEED DOCUMENTS PRINTED, YOU CAN EITHER GO TO THE LIBRARY OR EDUCATION CENTER

*A Medicare Card cannot be used as a form of ID when issuance of an ID card is being requested.

*If the two identity source documents bear different names, evidence of a formal name change must be provided.

DEERS

Initial DEERS Enrollment:
The sponsor must be present or information updated in ID Card Office Online or a current DD Form 1172-2 signed by the sponsor or a General Power of Attorney must be presented, and original or true court certified copies of required documents must be presented.  NOTE:  Only DD Form 1172-2 generated from the DEERS/RAPIDS system or MilConnect (Rapids Self-Service) can be utilized to produce 1172-2 for this office:  https://www.dmdc.osd.mil/self_service .
 
Enrolling a New Spouse:
Required documents - 
- Valid Federal or State picture ID
 
- Original or true certified marriage license
- Original birth certificate (can be the short form)
- Original Social Security Card
- All Divorce Decrees of the Sponsor/Spouse (if applicable)
- Disable American Veterans (DAV) - Award Letter showing dependents for all 100% and DD Form 214 Copy 4

DoD/Contract Employees:
Be aware, it may take up to 36 hours or longer to completely process into the RAPIDS/DEERS system.  NOTE:  NAF, AAFES and DoD School employees a DD Form 1172-2 is required for initial issuing of a CAC card.
 
Contractors: 
CAC information will be entered into DEERS through Contractor Verification System (CVS) or Trusted Associate Sponsorship System (TASS).  Please contact your POC to ensure that the information has been updated.
 
ID card Renewals:
Sponsors must be present or current DD Form 1172-2 signed by the sponsor or a General or Special Power of Attorney must be presented.  Original or certified true copies of required documents must be presented.
 
Sponsor or Spouse:
- Two forms of valid identification (not damage). One must be a valid Federal or State picture ID, and a birth certificate, social security card VA card, Voters Registration card or valid Passport)
- When turning age 65, a Medicare card with Parts A&B or a letter stating member is not eligible for Medicare is required.
 
Children: 
- Ages 10 to 17 - one form of identification (birth certificate or social security card)
 
Children:

College Age Family Members:
- Students aged 21-22 must provide two acceptable forms of identification that have not expired (a college ID is not an acceptable form of ID) along with a letter from their college registrar stating they are CURRENTLY enrolled FULL TIME pursuing an associate degree or higher. 
- Letter from the registrar's office or from National Student Clearinghouse (www.studentclearinghouse.org).  It must state that the student is a Full-Time student, with the current begin date and estimated graduation date listed.
- Students must be attending an accredited college or university. Students must have two forms of unexpired identification upon arrival (one must be current photo id). ALL DOCUMENTS MUST BE PRINTED AND BROUGHT WITH THE STUDENT TO THEIR APPOINTMENT.
 
Retirees:
 
Active Duty Retiree:
Required documentation:
- Final copy of DD Form 214 (copy 4)
- Retirement order
- Two (2) forms of valid identification (not damage).

DoD Civilian Retirees
In accordance with DoD memorandum dated February 17, 2023:
- DoD Civilian Retiree ID Cards are no longer issued.
- Previously issued DoD Civilian Retiree ID Cards are no longer valid as of August 31, 2023.
- DoD Civilian Retirees can enroll a REAL ID Act-compliant driver's license by submitting their retirement documentation at the installation Visitor Control Center (VCC) for access to the installation. 
 
Dependency Packages are available online only and must be submitted directly to DFAS at https://www.dfas.mil/militarymembers/SecondaryDependency/SDC.html
 
Available packets:  Parents, Parents-in-law, In Loco Parentis, Incapacitated Child (21 years or older), Student (21 or 22 years old) for BAH only, Ward (under 21 years old) for BAH only.
 
Types of disenrollment and required documents:
- Death of sponsor/spouse:  Death certificate
- Divorce:  Original or true certified copy of the final divorce decree. 
- Children under the age of 21 requesting removal from Sponsor’s DEERS Original or true certified marriage certificate, enlistment contract or Oath of Office or Court Order Emancipation

ADDING A SPOUSE:
*IF YOU ADD YOUR SPOUSE TO DEERS ONLINE THE INFORMATION MUST BE VERIFED AND COMPLETED BY A DEERS ID CARD FACILITY UPON COMPLETION. BE ADVISED THAT THE SPONSOR MUST BE PRESENT AT THE DEERS FACILITY ON THE DAY OF THE TRANSACTION.
*Original or true certified copied documents needed are:
* Birth Certificate (Passport may be substituted)
*Marriage Certificate
*Social Security Card,
*Valid Primary Form of Identification (check list of acceptable docs).
**NOTE:  If applicable, Divorce Decree, Annulment, or Original Death Certificate of previous spouses to remove them from the sponsor's profile in RAPIDS. (NOTE: If the spouse is 65 or older, then they will need to bring their Medicare Card). 
*If the last name of added spouse is not changed to married name during the initial requested service and wants to change their last name later, then they must provide two forms of ID with the same last name.
*ADDING A NEWBORN BABY/CHILD: (Per TRICARE, you have 90 days from the newborn’s date of birth to register them in DEERS)

**NOTE: 1172 MUST BE PRESENTED UPON ARRIVAL TO YOUR APPOINTMENT**
MINIMUM AGE FOR ID CARD:
**The minimum age for all ID card issuances requirement is age 10, unless dependent child/children are in a dual military/single/deceased sponsor or children of a divorce status. **
 RECENTLY RETIRED:
If you are on terminal leave, WE CAN OUT-PROCESS YOUR RETIREMENT 90 DAYS PRIOR TO YOUR OFFICIAL RETIREMENT DATE IN DEERS. Please stop by your nearest DEERS Office at the date of your Retirement/ separation or after. Ensure you bring a copy of your DD214 Member Copy 2 or 4 and retirement orders; you must surrender your CAC upon receiving your Retirement ID. Since your Status changed, all your dependents that have a current USID will need to receive a new one under your new status. ALL USID/CAC will need to be turned in and if authorized, you will receive new ones. Medicare questions

HOW DOES MEDICARE AFFECT YOUR MILITARY ID?

MILITARY RETIREES ONLY: 

*Your military ID card expires on the first day of the month when you turn 65 so you will need to make sure it has been renewed to keep your TRICARE eligibility.

*You can do this by enrolling in Medicare online at medicare.gov or by calling the SSA at 1-800-772-1213 or by visiting your local SSA office.

*You may apply 1-3 months prior to turning 65. Once you have your Medicare card provided by the Social Security Administration, please schedule an appointment NO EARLIER THAN 10 DAYS PRIOR TO EXPIRATION to have your ID card renewed.

*At your appointment, you will need to present your Medicare card and two acceptable forms of identification. 

SPOUSES OF RETIREES: 

*Your ID card will go to INDEF on your 65th birthday. 

*You will not be able to receive an INDEF card any earlier than that.  Once you turn 65 and have your Medicare card, you will be given an INDEF card. 

*If you are not 65 (even if you have Medicare), you will not receive an INDEF card.  You must be 65 years of age or the card you receive will take you out for 4 additional years until you provide your Medicare card.

100% DISABLED AMERICAN VETERANS (DAV):

100% DISABLED AMERICAN VETERAN (DAV) ELIGIBILITY DAVs must schedule an appointment prior to visiting the ID card facility.  Required Documents are:

 * 1. Your award letter from the VA shows your 100 percent total and permanent rating and the start date of your rating. Award letter should also state that there are no further examinations required.

 * 2. You may call VA Regional Office on 1-800-827-1000 for awards letter or go to va.go and search for a benefits letter and print from there (please print a copy of the letter to bring with you to your appointment).  ***NOTE: WE CANNOT ACCEPT THE PACKET SHOWING THE PERCENTAGE FOR THE CLAIMS THAT YOU SUBMITTED BECAUSE OF HIPPA. (This is the letter from the VA that you receive in the mailbox) *****

  *3. DD Form 214 (copy 2 or 4 ONLY -No other copy will be accepted-You may also use the Service copy of your 214) stating that the characterization of your discharge is Honorable. An Honorable discharge is required for veteran (who is rated 100 percent disabled) to qualify for DEERS enrollment and for ID card issuance. A General discharge (under honorable conditions) is considered an Honorable discharge. Therefore, veterans who are rated 100 percent disabled with a general discharge under honorable conditions qualify for DEERS enrollment, including their family members, or an ID card issued from RAPIDS. 

  *4. If you are married and you are enrolling your spouse in DEERS, you must have: * Your original marriage license or (certified true copy) * His/her birth certificate, or passport (cannot be expired) *Spouse's driver's license *Social security card.  **If you are enrolling children, you must have their original birth certificate with at least one parent name on the certificate and social security card.  **If you were married prior to your current spouse and not sure if former spouse is still on your record, please bring a copy of your divorce decree

  * 5. Finally, you will need a valid Primary and Secondary form of Identification (check list of acceptable documents). ALL DOCUMENTS MUST BE PRINTED AND BROUGHT TO YOUR APPOINTMENT AND EITHER THE ORIGINAL OR TRUE CERTIFIED COPY

LOST OR STOLEN IDS:

Two (2) valid, unexpired forms of identification must be presented, (18 and older)

Names must match (all documents must be original or true certified copy, and the primary IDENTIFICATION must be a valid unexpired state or federal issued photo ID).

ALL Service Members must bring a police report from the MP station, or a memorandum signed by their 1SG or Commander with the title of the counselor (i.e; 1SG/CDR).

Civilian employees must bring a memorandum signed by their supervisor or Division Chief and present the memorandum to the ID Card Facility for a new CAC.

Contractors must receive a signed memorandum from their Supervisor, TASS Agent, or Division Chief and present to the ID Card Facility for a new CAC.

ALL DOCUMENTS MUST BE PRINTED AND BROUGHT TO YOUR APPOINTMENT.

GROOMING STANDARDS:

ALL SERVICE MEMBERS REGARDLESS OF BRANCH AFFILIATION MUST present a profile, religious exemption or ETP for grooming standards when receiving a CAC (If applicable).

The policy will remain in effect while the service member is on leave.

**For appearance guidance refers to AR670-1, CH 3, Grooming Standards. **

ALL DOCUMENTS MUST BE PRINTED AND BROUGHT TO YOUR APPOINTMENT.

SERVICE MEMBERS NOT IN UNIFORM:

The I.D. Card facility does not require service members to be in uniform when obtaining their Military I.D. Card. 

NOTE: However, I.D. Card photos WILL NOT BE RETAKEN if service members (regardless of branch) chain of command requests it or due to undergoing a board for promotion.

Army personnel must keep in mind that their photo will be uploaded to their Soldier Talent Profile (STP) in IPPS-A.

 TSA DISCONTINUED USE OF DoD CREDITENTIALS:

From the Desk of the Deputy Project Officer/DEERS Lead

Effective May 7, 2025, TSA will no longer accept DoD ID credentials at checkpoints. Our DoD ID cardholders should be aware that these restrictions may affect their air travel.  When the restriction goes into effect, TSA will only accept the Common Access Card (CAC): Armed Forces of the United States Geneva Conventions Identification Card at TSA checkpoints.  Identity verification is essential to TSA's security model to determine passenger risk and appropriate screening. TSA uses Credential Authentication Technology devices to detect fraudulent IDs by verifying embedded security features. TSA cannot accept ID cards that have indefinite expiration dates. The above CAC is the only DoD ID that meets the necessary security requirements under Title 10, U.S. Code, Section 1060b (Note: not all CACs meet this requirement).  TSA explicitly stated that the anti-counterfeiting physical security features on current DoD ID credentials (e.g., microprinting, color shifting ink, and hologram) were not strong enough to meet TSA's recommended anti-counterfeiting features for other Department of Homeland Security/Department of State and state-issued REAL ID Act compliant ID cards.

Questions concerning the upcoming change should be directed at TSA.

DESIGNATED AGENT LETTER:

The Designated Agent letter is issued to an authorized person designated by the military sponsor (or surviving spouse).  This letter is only valid for one year at a time.

*Authorization cannot be granted solely for customer convenience.  An extreme medical hardship must exist and be validated.

*A parent who isn't the sponsor may be eligible for an Agent Letter, for children under 16 and whose sponsor is stationed away from the household.

*Dual military couples (active duty) and single parents (active duty) with children under 16 in the household may also have a designated agent when a Family Care Plan is in effect.

Required Documents:

-Authorized patron's presence or a valid Power of Attorney POA).  If the authorized patron is unable to be present at the time of issuance, the person obtaining the agent card (letter) must have a current POA authorizing them to conduct actions pertaining to the military on behalf of the sponsor or surviving dependent.

-Authorized patron's ID card

-Signed letters from attending physician on their letter head to include phone number: When the sponsor is physically unable to shop for themselves, they must obtain a letter from their doctor stating their incapacitating condition or inability to care for themselves.  The letter MUST state the full name of the designated agent.  (i.e., Army N. Military will be assigned as SGT Help's designated agent).  NOTE: The name must match the driver's license. This requirement does not apply to active-duty military couples or active-duty single parents with children.

-Original Birth Certificate of sponsored minor under age 16.

ACTIVE DUTY ONLY:

*Copy of current Family Care Plan: Dual military couples or single parents with children must provide this to obtain a Designated Agent letter (usually in cases of deployment, unaccompanied PCS or TDY).  The designated agent must be someone designated as a caregiver on the family care plan.

*Designated agent ID with photo required:  Driver's License or a state/government issued photo ID; preferably valid driver's license.
