ID Card Eligibility

ID Card Eligibility Requirements

Proof of Identity is Mandatory! Below is guidance for acceptable identification documents.

  • Two (2) valid, unexpired forms of identification must be presented (Over the age of 18) to receive any ID card. For a list of authorized proof of identity please see Department of Defense (DoD), Lists of Acceptable Documents.
  • All documents must be originals, certified or notarized copies.
  • All foreign documents must be originals with an Apostille and translated in English. Please contact the department of Foreign Ministry Affairs in the country of issuance or Google "Apostille certification & country" to obtain guidance.
  • All court documentation must be state or county certified stating that it has been filed and recorded in the courthouse.
  • Driver's Licenses must have an actual date of expiration.

 

Documents needed to issue ID Card

Military Member

  • Official documentation must be presented for any updates or changes (i.e. Promotion Orders, Name Change Memorandum, Medicare Card, Official Notification Letter from Social Security Administration, etc)
  • Must present two (2) valid forms of identification.


DoD Civilian

  • All data must be previously input through Civilian Personnel Advisory Center (CPAC).
  • Must have two (2) valid, unexpired forms of identification

DoD Contractor

  • All data must be previously input through Trusted Agent Sponsorship System (TASS).
  • If going overseas, a Letter of Authorization (LOA)/Synchronized Pre-deployment and Operational Tracker (SPOT) is required. For initial card, LOA cannot be older than 30 days.
  • Must have two (2) valid, unexpired forms of identification
 

Dependent(s)

  • Must have two (2) valid, unexpired forms of identification and one of the following:
    • Sponsor must be present
    • Valid Power of Attorney (POA)
    • DD Form 1172-2: ID Card Application Card Application signed by the Sponsor and a Verifying Official
  • DD Form 1172-2: ID Card Application signed by the Sponsor and a Notary Public located at DD Form 1172-2 ID Card Application

 

Documentation needed to add Family members to your DEERS Record

Spouse

    • Marriage Certificate
    • Drivers License  (unexpired)
    • Birth Certificate 
    • Social Security Card
    • Photo ID issued by state or local government
    • Final Divorce Decree, annulment or death certificate from previous marriage

 

Former Spouse

  • Driver’s License
    • Military ID Card (DD Form 1173)
    • Photo ID issued by state or local government
    • Social Security Card

 

Children

  • If parents are not married, see Child born out of Wedlock section below
    • Birth Certificate (Must contain the name of at least one parent) or Hospital Confirmation of Birth
    • Social Security Card (if received)

 

Step Children

  • Marriage Certificate
  • Birth Certificate must contain the name of at least one parent) or Hospital Confirmation of Birth
  • Social Security Card

 

Adopted Children

  • Final Adoption Decree or documentation from authorized placement agency under the Department of Human Services
  • Birth Certificate
  • Social Security Card

 

Ward

  • Court order granting custody to the Sponsor. Court order must stipulate that the child will be in the care and custody of the Sponsor for no less than twelve (12) consecutive months. The court order can designate length of custody by age, time or permanency.
  • Birth Certificate
  • Social Security Card

 

Child Born Out of Wedlock

  • Birth Certificate
  • Social Security Card
  • One of the following:
    • Voluntary Acknowledgment of Paternity from the State in which the child was born
    • Court Order establishing paternity

 

Child Born Overseas

  • FS 240: Consular Report of Birth Abroad or DS Form 1350: Certification of Report of Birth
  • Social Security Card

 

Incapacitated Child 21 or over (Army Only)

  • DFAS approval if applicable.
  • Photo ID
  • Military ID
  • SS card
  • Medical sufficiency statement (memorandum from doctor).

Must be re-certified every 4 years

Parent/Parent-in-law

  • DFAS approval
  • Photo ID
  • SS card
  • Birth certificate of Parent/Parent-in-law
  • Birth certificate of sponsor and/or spouse
  • Marriage certificate of sponsor and spouse.
  • NOTE:  The ID Card Facility does not handle the approval for these packets. Dependency determination may be done electronically.  To submit a packet, visit  https://corpweb1.dfas.mil/askDFAS/custMain.action?mid=5340, and choose “Submit a Ticket” or mail the packet to:

Retirees (Initial Retirement)

  • Initial Retirement: Retirement Orders and DD Form 214 (Copy 4)
  • Reserve/National Guard Retirees need Orders/Letter transferring to Retired List and authorizing retired pay
  • If Medicare eligible, must present Medicare Card
  • Must have two (2) valid, unexpired forms of identification


100% Service Connected Disable Veteran (DAV)

  • Letter from Department of Veteran Affairs showing member as an honorably discharged veteran rated as 100%  service connected, authorizing commissary store and exchange privileges
  • DD Form 214 (Copy 4)
  • Must have two (2) valid, unexpired forms of identification.
  • Spouse and Children will need to provide documentation under Family Member Information


Full-Time Students (21-23 years old)

  • Letter from Student Registrar's Office or National Clearing House declaring full time student status with an expected date of graduation. If no graduation date, a letter will be required for each semester to maintain benefits.  Must be an accredited school. 
  • Sponsor must be providing over 50% support - must be stated in block 89 (Remarks) of the DD 1172-2
  • Must have two (2) valid, unexpired forms of identification
