Required documents

Note: Sponsor must be present for all transactions unless current and valid Power of Attorney is presented. Federal law does not recognize expired IDs as valid. Even if the state or issuing entity extends its validity, such as extending driver’s licenses, due to being in the military. If the U.S. government extends the validity then the ID will be recognized as valid during the published time.

ID Card Renewal/Update

Active Duty, National Guard, Reserve

1. Two forms of valid ID
a. Old CAC with driver’s license and Social Security card birth certificate or valid passport

2. The following applicable documents:
a. Promotion order
b. Reenlistment contract or extension orders
c. Enlisted Record Brief (ERB)
d. Officer Record Brief (ORB)
e. Oath of Office (from enlisted to officer)

3. Military members must be within grooming standards, per AFI 36-3026

4. ID cards are not reissued to Soldiers or Family members being frocked until promotion orders are presented.

5. Active-duty Soldiers name change: ERB or ORB and updated driver’s license or Social Security card.

6. Reserve/National Guard name change: 2-A for enlisted/2-B for officers.

Active-Duty Retirees

1. Two forms of valid ID
a. Old CAC with driver’s license and Social Security card, birth certificate or valid passport

2. DD 214 (copy 4) or retirement orders

Reserve Retiree (60 and older)

1. Two forms of valid ID
a. Old Reserve retiree ID card with driver's license and Social Security card, birth certificate or valid passport

c. Expected date of graduation
d. Must state full-time or at least 12credit hours
e. Point of contact name, phone number and signature of school official.

2. Old ID, valid state or federal ID with Social Security card

Note: Sponsor must provide over 50% of dependent's care.

Mail Out Program

1. Sponsor resides in another state and no Power of Attorney is available
a. Two forms of valid ID: Old ID card with driver's license and Social Security card, birth certificate or valid passport
b. Current address of sponsor.

Note: Dependent will beissued 30-60 day temporary IDuntil mail-out returned.

a. Notarized 4x6 (minimum size) photo of applicant
