Total Army Sponsorship Program (TASP) (DA Form 5434/ Survey) MANDATORY for (PVT-SSG/2LT-CPT/WO1-CW2); OPTIONAL for SFC-CSM/CW3- CW5/MAJ-COL. **PCS ONLY***

Total Army Sponsorship Program (TASP)
Applies to PVT-SSG, W1-CW2, 2LT-CPT are required to participate in the Sponsorship program, except those on assignment to a PCS/TDY length school (more than 20 weeks).  Soldiers in higher ranks may request a sponsor if so desired.

Please go to https://actnow.army.mil and complete the Total Army Sponsorship Program (TASP) DA Form 5434 and survey. Complete sections 1, 2, 4, & 5. After you have completed all sections, please check I CERTIFY block; add your rank in Section 1 and save your form at the bottom of the page.  Additionally, complete the TASP Survey.
