Demand Maintenance Order (DMO)/Service Order
Can't get it done through the Self Help Store (see information below)? Give us a call or send an e-mail to work reception for minor repairs and service orders. You can also submit a Demand for Maintenance Order online:

MAJOR REPAIRS
For major work that is beyond the scope of a demand maintenance order, such as a renovation project, expanded self-help project or proprietary approval, please submit a Facilities Engineering Work Request (DA4283) and a Request for Environmental Review (AE200-1A) to work reception by email.
In order for DPW to better assist you, please provide enough details to fully describe your requirements and include a valid justification for project approval. Attach additional documentation (i.e. Request for Environmental Review, pictures, drawings, other pertinent documents) as needed.
As per USAG Benelux Regulation 200-1, project proponents will submit a Request for Environmental Review together with a Facilities Engineering Work Request. Please fill out AE200-1A lines 1 to 9 and submit with DA4283.
