OUR MISSION
It is our mission to ensure enrollment in the Defense Enrollment Eligibility Reporting System (DEERS) and provide Common Access Cards (CAC) or Official Identification Cards to eligible Family members, DOD Civilians, Retirees, and Contractors working on USAG Humphreys. These services are also available to Non-PSDR Soldiers who do not have a servicing Brigade or HHBn.
PSDR Soldiers are required to use their Brigade/HHBn/STB S1 for any CAC related issues. See 8A OPORD 21-01-19-01.

ID Card
OUR BASIC PROCEDURES
Customers must present valid identification and required documentation at the time of visit. Sponsors must accompany their Family members at all times. However, this facility does accept power of attorney in the absence of the Sponsor, or a DD Form 1172-2 that is generated by a DEERS/ID Cards Facility or online. The form has to be generated within 30 days prior to your appointment.
All individuals requesting a replacement for a lost or stolen CAC are required to present documentation from their local security office or CAC sponsor, confirming that the CAC has been reported lost or stolen. The individual will still be required to produce 2 forms of ID to acquire a new CAC.
DOCUMENTS REQUIRED FOR ID CARD ISSUANCE
COMMON ACCESS CARD (CAC): Reserve, Guard, Civilian, NAF/AAFES, and Contractor
- Army Enterprise Email Address (army.mil) if DA Civilian (Not required for CAC issuance).
- Contractors must be verified in CVS every time a card is issued.
- Two valid forms of ID
GENEVA CONVENTION CAC
- Civilian: Copy of orders generated and completed through DTS or a completed DD Form 1610
- Contractor: Must have a SPOT LOA (Letter of Authorization) with the proper boxes checked
RETIRED MILITARY
- DD Form 214 or Retirement Orders
- Two valid forms of ID
INDIVIDUAL READY RESERVE (IRR)
- DD Form 214

DEPENDENT ID: Sponsors must accompany their Family members at all times. However, this facility does accept power of attorney in the absence of the Sponsor or a DD Form 1172 that is generated by a DEERS/ID Cards Facility within the last 90 days.
- Spouse: Two valid forms of ID and Medicare Card if 65 years or older
- Un-remarried Widow: Two valid forms of ID and Medicare Card if 65 years or older
- Former Spouse: Two valid forms of ID and Medicare Card if 65 years or older
- Dependent younger than 18: No ID necessary
- Dependent older than 21 (Student): Two valid forms of ID, letter from school registrar office stating full-time enrollment (verification from clearing house also acceptable), and the expected graduation date at accredited college
- Dependent older than 21 (Incapacitated): Two valid forms of ID, current dependency determination* and medical sufficiency letter
- Other Dependent (Parent, Parent-in-Law, etc.): Two valid forms of ID and current dependency determination*
*Dependency determination must be completed every four years or status change.
DISABLED VETERAN (INITIAL ISSUANCE)
- Letter from Veterans Affairs stating member is 100%
- Two valid forms of ID

In order to use any of the duty-free facilities (Exchange, Commissary, Shoppette, etc.), each DOD ID Card holder must be properly registered with DBIDS.
To process any ration control request, please bring the following documents:

Secondary Dependency
SECONDARY DEPENDENCY APPLICATIONS
To be considered sufficiently dependent upon the Sponsor, the claimed dependent's qualifying income must be less than 50% of his/her expenses and the member's contribution must exceed 50% of the dependent's expenses. This can include:
- Parent, parent-in-law, Stepparent, parent by adoption, or any person who stood in loco parentis to the member for at least five years prior to the member's emancipation.
- Ward of a court who is unmarried and placed in legal and physical custody of the member for a period of at least 12 consecutive months.
