Extended Policy Guidance for USID cards
Extended policy guidance for the Uniformed Services ID (USID) that expired between January 1, 2020 and July 31, 2021 only, through:
October 31, 2021 for dependents of Active Duty Uniformed Service members, and for Reserve and National Guard Uniformed Service members and their dependents.
January 31, 2022, for Retirees and their dependents, and all other populations.

DEERS/ID Cards

General Information

For information on hours of operations, scheduled closures, and contact information for the DEERS/ID Card Office, please see the "Hours and Contact Information" tab.

Customers must present 2 valid forms of identification and the appropriate required documentation to receive ID card services.

  • Sponsors must accompany their family members at all times. However, in the absence of the sponsor, a power of attorney or an electronically generated DD Form 1172-2 is acceptable. The DD Form 1172-2 can be generated by the DEERS/ID Card Office or online by visiting https://idco.dmdc.osd.mil/idco/
  • All individuals requesting a replacement for a lost or stolen CAC must present documentation from the Police Station or Sponsor confirming that the CAC was reported lost or stolen.
  • Acceptable IDs

    Two valid forms of identification are required to receive a government issued ID

    One valid form must be from the "primary proof of identity" list, the second form can be from either list. The identity source documents must be bound to the applicant and shall neither be expired nor cancelled. If the two identity source documents bear different names, evidence of a formal name change must be provided.

    Original or certified copy of a Birth Certificate issued by a state, county, municipal authority, possession, or outlying possession of the United States, bearing an official seal

    Driver's License or an ID Card issued by a state or possession of the United States, provided it contains a photograph

    ID card issued by a federal, state, or local government agency or entity, provided it contains a photograph

    U.S. Passport or U.S. Passport Card

    Voter's registration card

    Permanent Resident Card or an Alien Registration Card (INS Form I-551)

    U.S. Coast Guard Merchant Mariner Card

    Employment Authorization document that contains a photograph (INS Form I-766)

    Cerrtificate of U.S. Citizenship (INS Form N-560 or N-561)

    Personal Identity Verification Card

    Certificate of Naturalization (INS Form N-550 or N-570)

    Foreign Passport

    U.S. Citizen ID Card (INS Form I-197)

    ID Card for Use of Resident Citizen in the United States (INS Form I-179)

    Certificate of Birth Abroad or Certification of Report of Birth issued by the Department of the State (Form FS-545 or Form DS-1350)

    Temporary Resident Card (INS Form I-688)

    Employment Authorization Card (INS Form I-688A)

    Reentry Permit (INS Form I-327)

    Refugee Travel Document (INS Form I-571)

    Employment Authorization document issued by the Department of Homeland Security

    Employment Authorization document issued by the Department of Homeland Security with photograph (INS Form I-688B)

    - Contractors must be verified in TASS each time a card is issued

    Geneva Convention CAC

    - Contractor must have a SPOT LOA (Letter of Authorization) with the proper boxes checked
    - Civilians need a copy of their orders generated and completed through DTS or a completed DD Form 1610

    Retired Military

    - Two valid forms of ID (see "Acceptable ID" tab)
    - DD Form 214 or Retirerment Orders

    Individual Ready Reserve

    - Letter from School Registrar Office stating full time enrollment (verification from Clearing House also acceptable), expected graduation date, and the dates of the current quarter or semester currently enrolled in

    Dependent over 21 years of age (Incapacitated Child)

    - Two valid forms of ID (see "Acceptable ID" tab)
    - Current Dependency Determination (must be completed every 4 years or on status change
    - Medical Sufficiency Letter

    Other Dependent (Parent, Parent-in-Law, etc)

    - Two valid forms of ID (see "Acceptable ID" tab)
    - Current Dependency Determination (must be completed every 4 years or on status change)

    DAV (initial issuance)

    -Two valid forms of ID (see "Acceptable ID" tab)
    -Letter from VA stating member is 100% disabled

    Update/Verify DEERS

    Online:
    Visit the DEERS Web Site at: https://idco.dmdc.osd.mil/idco/ and follow the steps to update your address, email address, and phone numbers online. (MUST have CAC or DFAS PIN)
    Via the Beneficiary Web Enrollment Web Site at: https://idco.dmdc.osd.mil/idco/ This site is linked directly to the DEERS database, so when you update your information via this portal, it not only updates DEERS, but also with your regional contractor. (MUST have a CAC or DFAS PIN)

    Telephone:
    Call the Defense Manpower Data Center Support Office at 1-800-538-9552 to update your addresses, email address, and phone number. (TTY/TDD for the deaf: 1-866-363-2883)

    Postal Service:
    Mail changes to the Defense Manpower Data Center Support Office. You must include supporting documentation if you are updating a Family member’s status:

    Dependency Applications

    To be considered sufficiently dependent upon the Sponsor, the claimed dependent’s qualifying income must be less than 50% of his/her expenses and the member's contribution must exceed 50% of the dependent's expenses. This can include:

    • Parent, parent-in-law, step-parent, parent by adoption, or any person who stood in loco parentis to the member for at least five years prior to the member's emancipation.

    • Ward of a court who is unmarried and placed in legal and physical custody of the member for a period of at least 12 consecutive months.

    • When the Sponsor is Army, dependency determination for Wards of Court is not required
