If you need a new ID card or need to update your info in DEERS:

You must bring with you TWO (2) Forms of Identification issued by state or federal; 1 of the 2 ID must be a photo-ID.

SPONSOR MUST BE PRESENT FOR ALL TRANSACTIONS.  IF SPONSOR IS NOT PRESENT, DEPENDENT MUST HAVE A CURRENT AND VALID GENERAL/SPECIAL POWER OF ATTORNEY,  A COMPLETED DD FORM 1172-2 GENERATED IN MILCONNECT WEBSITE (https://idco.dmdc.osd.mil/idco) WITH THE SPONSOR'S DIGITAL SIGNATURE,  OR A COMPLETED DD FORM 1172-2 HAND-SIGNED IN FRONT OF A VERIFYING OFFICIAL FROM AN ID CARD FACILITY.  THE FORM MUST HAVE STATEMENTS IN THE REMARKS SECTION AUTHORIZING DEPENDENT THE REQUIRED DEERS TRANSACTION WITHOUT THE SPONSOR PRESENT.

CAC RENEWAL/UPDATE for Active Duty, National Guard, and Reserve Service Members

  • Old Common Access Card (CAC)
  • The following documents as it applies:
    • Reenlistment Contract/ Extension
    • Promotion Orders
    • Active Duty Orders 
    • Oath of Office (DA FM 71)

DOD CIVILIAN OR NON-APPROPRIATED FUNDS (NAF) EMPLOYEE

DISABLED AMERICAN VETERANS

  • VA Letter in memorandum format stating 100% combined or overall rating of disabilities OR 100% unemployable.

RETIREE

  • Standard Form 50 (SF-50) Personnel Actions - stating DoD Civilian is retired and/or receiving retired pay.

 

NEW MARRIAGE

  • Marriage Certificate (Original or Certified Copy)
  • Valid State issued Photo ID or Birth Certificate
  • Social Security Card or Letter from SSA with SSN# and Name
  • Final Divorce Decree, if prior marriage

 

NEW (BORN) CHILD/STEPCHILD

  • Birth Certificates - includes at least one parent’s name.
  • OR Confirmation of Birth signed by any attending physician or medical personnel.
  • Social Security Card or Letter from SSA with SSN# and Name (if it is already available).

NOTE: Child less than 10 yrs old may not received an ID Card. Exemptions are:

  • Single Parent
  • Dual Military Parents
  • Child residing outside the Sponsor Household

 

LEGAL CUSTODY/WARDS

  • Final Legal Custody, Guardianship Court Order for at least 12 consecutive months.
  • Birth Certificate (Original or State Certified)
  • Social Security Card or Leter from SSA

 

CHILD BORN OUT OF WEDLOCK

  • Court Order Establishing Paternity or Approved Dependency Determination or Voluntary acknowledgement of Paternity:

DEPENDENT CHILDREN AGE 21 - 22:  to continue eligibility in DEERS, dependent child must be/have:

    • Must state expected Date of Graduation
    • Must state Full Time or at least 12 Credit Hours
    • Must be on official Letterhead from Accredited School
    • POC, Name, Phone Number, Signature of Registrar

OR

  • Enrolled in Tricare Young Adult (TYA) and the enrollment status has reflected in DEERS. Tricare Office is responsible for the child's TYA enrollment in DEERS; not the ID card office.

 

DEPENDENT CHILDREN AGE 23 - 26:  To continue eligibility in DEERS, dependent child must be/have:

  • A Student
  • Letter From The School Registrar OR An Acceptable Third Party with the following Information:
    • Must state expected Date of Graduation
    • Must state Full Time or at least 12 Credit Hours

  • Child: Birth Certificate (FS 240 or DS FM 1350)
  • NOTE: All documents must be translated in English

 

OTHER ELIGIBLE DEPENDENTS

Parents or Parents In-Law

  • Approved Dependency Determination
  • Birth Certificate or State Issued Photo ID

UNREMARRIED FORMER SPOUSE

YOU MUST MEET ALL THE CRITERIA. ALL DOCUMENTS MUST RECEIVED BEFORE PACKET CAN BE FORWARDED TO THE DEERS PROJECT OFFICE. PLEASE CLICK HERE TO PRINT THE REQUIRED DCOUMENTS FROM FORMER SPOUSE.

  • Final Divorce Decree (for initial ID Card only) (must include the first page, the page that showed the marriage was dissolved, and the page with the Judge's signature and date)
  • Dependent ID Card, valid Federal or State-issued photo ID, AND Social Security Card
  • DD 214 or Statement of Service (from the Sponsor's unit if Sponsor is still serving)
  • Death Certivicate (if applicable)
  • Signed Former Spouse written statement (ID Card Office can provide this statement to be completed and signed)

CRITERIA:
Only Spouse with at least 20 years of marriage or more, 20 years or more service creditable eligibility for retired pay, 15 or 20 years or more years of marriage must overlap during the member performed creditable service may receive benefits.

If remarries will loses ID Card and entitlement. If a former spouse becomes unmarried through divorce, annulment or death, he/she may be eligible for reinstatement of ID Card for Commissary, Exchange and MWR only.
