PROOF OF IDENTITY IS MANDATORY
- Two valid, unexpired forms of identification must be presented to receive any ID card
- All documents must be originals, or they must be certified or notarized copies
- All foreign documents must be originals with an apostille and translated into English
- All court documentation must be state or county certified, stating that it has been filed and recorded in the courthouse
- When a Social Security Number is required, you may use a Social Security card or other official documentation showing the SSN (i.e. Tax ID form, Military ID, letter from the Social Security Office with SSN)
- Driver's licenses must have an actual date of expiration
LOST OR STOLEN ID CARDS
Two acceptable forms of valid identification (federal or state-issued) are required to receive a replacement ID. Identification can be two forms of picture ID, or one picture ID plus birth certificate or Social Security card.
LOST OR STOLEN COMMON ACCESS CARDS (CAC)
A memorandum for the lost or stolen Common Access Card plus two acceptable forms of valid idenfitication (federal or state issued) are required to receive a replacement.
**Military Members: A memorandum from the Service Member's commander, first sergeant, or adjutant confirming that the CAC has been reported lost or stolen, or a counseling statement
**DOD/DA Civilian employee: A memorandum from the employee's HR department or supervisor describing the date, place and circumstance of the loss or theft.
**DOD Contractor personnel: A memorandum from the contractor's Trusted Agent (TA) describing the date, place and circumstances of the loss or theft.
