If YES: Go to question 3.

If NO: Consult with company leadership to establish plan. Consider consultation with BHO and chaplain. Go to question 4.

3a. Do you intend to act on these thoughts of killing yourself? 

3b. Have you felt that way (acting on thoughts of killing yourself) at any point in the past month?

If YES: High risk. At a minimum escort to Company leadership to establish plan and consult with BHO or Chaplain. If BHO and Chaplain unavailable, escort to EBH or if after duty hours, escort to Emergency Department.

If NO, escort to company leadership to establish plan. Consider consultation with BHO and chaplain or escort to EBH/emergency department.

4. Have you started to work out specific details of how you would kill yourself?

If YES in past 3 days: High risk. Escort to EBH, BHO or chaplain. If after duty hours, escort to emergency department. If not physically present (i.e. talking with Soldier on the phone), call 911 for transport to ED. Inform company leadership.

If NO, or YES more than 3 months in the past: Inform company leadership. Ask about access to personal firearms or other lethal means and confirm that any personal firearms are stored safely. Go to question 5.5

5. Have you taken any steps to prepare to kill yourself? 
Examples:  collecting pills, obtaining a gun, giving away valuables, writing a will or goodbye letter, practicing holding a gun, tying a noose, cutting yourself.

If YES in the past 3 days: High risk. Escort to EBH, BHO or chaplain. If after duty hours, escort to emergency department. If not physically present (i.e. talking with Soldier on the phone), call 911 for transport to ED. Inform company leadership.

 

Carson CARES Battle Drill

The Battle Drill provides step-by-step guidance on how to navigate situations where a service member is distressed from environmental stressors or suicidal ideation.

  • Step 1: Witness identifies a stressed service member who exhibits warning signs of suicidal behavior.
  • Step 2: Witness exercises knowledge from ACE Intervention training and ACE card to determine if stressed service member has had thoughts of suicide. The witness will Ask, Care and Escort as trained.
  • ​​Step 3: Thoughts of suicide by a stressed service member requires company/troop/battery command leadership intervention. A stressed service member with thoughts of suicide will be safeguarded and cared for by the witness until they are safely escorted to their company leadership.
    • Service members acting out preparation of suicide or in the act of suicide requires immediate medical attention and 911 should be called immediately.
    • Service members with suicidal thoughts who are unable to get escorted to their company leadership will be escorted to a chaplain, behavioral health or ER.
  • Step 4: Witness does a proper handoff, sharing all precluding information, to ensure that care continues properly.
  • Step 5: Command leadership CARES for stressed service member by engaging with empathetic communication.
    • Ensure the stressed service member understands that they are not a burden and their leadership is committed to assisting them with overcoming or coping with their environmental stressors.
  • Step 6: Command leadership builds rapport and uses open-ended fact-finding questions to determine what environmental factors are impacting the stressed service member.
    • It is common for stressed service members to have more than one environmental stressor impacting them.
  • Step 7: Command leadership uses the Carson CARES Resource Guide to develop a plan that will help address applicable environmental factors impacting the stressed service member.
    • Company/troop/battery commander is required to approve resource plan on a Memorandum for Record (MFR).
  • Step 8: Command leadership will incorporate the use of the Decision Matrix to assist with identifying risk level potentially requiring behavioral health assistance or intervention.
    • Command leadership should seek guidance from their unit Behavioral Health Officer or chaplain as needed.
    • Service members will never be denied any requests to see Behavioral Health.
  • Step 9: Witness or trusted leader is assigned as a Carson CARES mentor to ensure the stressed service member has assistance with the outlined resource plan developed for them.
    • Team leaders or squad leaders are good options to assist service members.
  • Step 10: Command leadership continues to follow up on the stressed service member to ensure that the resource plan established is alleviating pressure from environmental stressors.
    • Command leadership will keep higher command informed as needed.

  • Fort Carson C.A.R.E.S Form section will display in the upper left of the Power App landing page. Populate all fields in the form with the same SIR/CCIR related information as you entered for the SIR/CCIR document.
    • Download the memo format from the top left of the CARES form for use in authoring the CARES memo which will be sent out to command teams and the Carson CARES team in step 5. 
    • ALL FIELDS MUST BE FILLED OUT.  Use accurate, detailed, or best judgement when populating information. “Unknown” and “NA” are allowed but should be used sparingly.
    • The form is large so you may have to scroll down to see the rest of the form. Summary Fields are for narratives and detailed plans of action.
    • Note: Click here to locate and identify resources that will be used for the subjects CARES Plan. Sponsor, Command, and Point of Contact must be accurate with phone number.
    • NOTE: Prior to completing step 3 ensure all information entered into the webform is entered into the memo which was downloaded
  • Step 3: Once ALL fields are complete, scroll to the top of the form and click “Submit C.A.R.E.S Plan.”
  • Step 4: Form will clear to verify submission. 
  • Step 5: Once an O6-level or higher commander has approved the CARES plan, submit the plan to usarmy.carson.4-id.mbx.carson-cares@army.mil.

Counseling - Military and Family Life Counselors (MFLC)

Services offered through the Military and Family Life Counseling Program are confidential, not reported to the command, and do not impact a service member’s security clearance. 

MFLCs can help you manage life challenges such as:

The military offers enhanced support to service members who have special needs Family members through the Exceptional Family Member Program (EFMP). Army Emergency Relief (AER) - Financial Assistance

Army Emergency Relief (AER) is a non-profit organization that is dedicated to “Helping the Army Take Care of its Own.” AER provides financial assistance to active and retired Soldiers, their family members, surviving spouses and orphans who are experiencing a financial hardship. Army Reserve, National Guard, and their Family members are eligible when on continuous active duty under Title 10 orders for at least 30 consecutive days.
