The DPW work order section accepts work orders (DA 4283) which must be submitted in person. An online form DA 4283 is provided for your download and use. Work orders are submitted for new work, major repairs/modifications, and minor repairs above a service order level. A work request is work that is beyond the scope of a service order. It requires a combination of trades and usually require greater than 32 hours of work. Please use the link to "save" (not "open") the electronic form to your desktop or other desired drive.

The DA Form 4283 Facility Engineer Work Order is the only document that permits DPW to do work. Work orders have to be submitted by the Director or Commander or an authorized requestor, clearly written and have correct information, justification, POC, etc, and approved by the DPW. Work orders cannot be called in or submitted electronically and must be prepared by filling out the DA Form 4283 and turned in to DPW Work Order Desk in person. Work orders are not for furniture, fixtures, equipment, telephone or LAN lines.  The DPW Work Reception can tell you if the work you are requesting requires a DA Form 4283.
