PACKET CONTENTS

FORMAL TRAINING WAIVER:

Used in lieu of attending an Army Aviation MOS-Q formal training IAW DA PAM 611-21 and AR 601-210. An MOS Crosswalk and ICTL Evaluation is required. Additional documentation needs will vary, for more information, please contact the OPFD Mailbox at: usarmy.rucker.avncoe.mbx.atzq-cdf-p@army.mil

  • DA Form 4187 or IPPS-A PAR (saved or ”printed” as PDF) requesting “re-classification” signed by both the Company and Battalion Commanders (no need to complete a separate 4187 requesting a grade/rank waiver)

  • Current IPPS-A Soldier Talent Profile (STP) and/or Enlisted Record Brief (ERB) (validates assignment history)

  • At least one of the following:

    • Aviation Course Completion Certificate

    • DD Form 214

  • Secret clearance verification memorandum signed by unit S-2/security manager (only applicable for MOSs 15C, 15E, 15M, 15P, 15Q, 15W, & 15Y)

  • Most current signed DD 2808 medical exam (validating PULHES and color vision) OR Class IV medical exam for MOSs 15C, 15Q, and 15W

  • DA Form 3340 Request for Continued Service or Reclassification in the Regular Army

  • Orders/graduation certificate/”pink card” of Rate/AFSC/civilian equivalent profession equivalent to Army MOS requesting

 

OPFD processes waiver requests within 30 days of receipt of all required documents. OPFD will forward final disposition of waiver requests to Packet POC for distribution. OPFD will reach out to the Packet POC for necessary documents as needed; packets that go 2 weeks without Packet POC communication will be marked incomplete and OPFD actions discontinued.
