You must bring with you TWO (2) Forms of Identification issued by state or federal; 1 of the 2 ID must be a photo-ID.
SPONSOR MUST BE PRESENT FOR ALL TRANSACTIONS. IF SPONSOR IS NOT PRESENT, DEPENDENT MUST HAVE A CURRENT AND VALID GENERAL/SPECIAL POWER OF ATTORNEY, A COMPLETED DD FORM 1172-2 GENERATED IN MILCONNECT WEBSITE (https://idco.dmdc.osd.mil/idco) WITH THE SPONSOR'S DIGITAL SIGNATURE, OR A COMPLETED DD FORM 1172-2 HAND-SIGNED IN FRONT OF A VERIFYING OFFICIAL FROM AN ID CARD FACILITY. Enrolled in Tricare Young Adult (TYA) and the enrollment status has reflected in DEERS. Tricare Office is responsible for the child's TYA enrollment in DEERS, not the ID Card Office.

UNREMARRIED FORMER SPOUSE
YOU MUST MEET ALL THE CRITERIA. ALL DOCUMENTS MUST RECEIVED BEFORE PACKET CAN BE FORWARDED TO THE DEERS PROJECT OFFICE. PLEASE CLICK HERE TO PRINT THE REQUIRED DCOUMENTS FROM FORMER SPOUSE.
- Final Divorce Decree (for initial ID Card only) (must include the first page, the page that showed the marriage was dissolved, and the page with the Judge's signature and date)
- Dependent ID Card, valid Federal or State-issued photo ID, AND Social Security Card
- DD 214 or Statement of Service (from the Sponsor's unit if Sponsor is still serving)
- Death Certivicate (if applicable)
- Signed Former Spouse written statement (ID Card Office can provide this statement to be completed and signed)
CRITERIA:
Only Spouse with at least 20 years of marriage or more, 20 years or more service creditable eligibility for retired pay, 15 or 20 years or more years of marriage must overlap during the member performed creditable service may receive benefits.
If remarries will loses ID Card and entitlement. If a former spouse becomes unmarried through divorce, annulment or death, he/she may be eligible for reinstatement of ID Card for Commissary, Exchange and MWR only.
