Services Offered at this Office
ALL Active-Duty Soldiers assigned to 25th ID, TSC, 500th MI, USARPAC MUST see their S-1 for assistance. All others should schedule an appointment online from the link in the hours of operation box above.
- CAC issuance/re-issue
- Effective 25 July 2024, ALL personnel renewing a CAC MUST bring two(2) unexpired forms of identification NOT including your CAC.
- Renewal of all ID Cards are issued within 90 days of expiration per DoDM 1000.13-M-V1, July 28, 2020.
- DEERS enrollment (ex. Spouse, newborn)
- Pin resets – can be done on a walk-in during business hours
- CAC Certificate updates

ID Card Eligibility Requirements
Proof of identity is MANDATORY! Below is guidance for acceptable identification documents:
- Two (2) valid, unexpired forms of identification must be presented (over the age of 18) to receive any ID card. For the list of authorized proof of identity please see Department of Defense (DoD), Lists of Acceptable Documents.
- All documents must be original or certified copies, NOT notarized.
- All foreign documents must be originals with an Apostille and translated English. Please contact the Department of Foreign Ministry Affairs in the country of issuance.
- All court documents must be state - or county-certified stating it has been filed and recorded in the courthouse.
- Driver’s licenses must have an actual expiration date or be accompanied with document from DMV stating it is valid upon expiration due to the military.
Documents Needed to Issue ID Card
Military Member:
- Must present two (2) valid, unexpired forms of identification. Please see “Department of Defense (DoD), Lists of Acceptable Documents” above.
Dependent(s):
- Must have two (2) valid (if over the age of 18, one (1) form if under the age of 18), unexpired forms of identification AND one of the following:
- Sponsor must be present
- Valid Power of Attorney (POA)
- DD Form 1172-2; ID Card Application signed by the Sponsor and Verifying Official or DD Form 1172-2 signed by the Sponsor and Notary Public located on the DD Form 1172-2 Application.
Documentation for Enrollment
Spouse:
- Original Marriage Certificate
- Driver’s License (unexpired) or any photo ID issued by state or local government
- Original Birth Certificate
- Original Social Security Card
**Note: Original Divorce Decree is required to remove spouse from your record**
Children:
- If parents are not married, see Child Born out of Wedlock, right below
- Original Birth certificate (must contain the name of at least one (1) parent
- Social Security Card (if received, upon receipt of Social Security card, it must be updated in DEERS record)
Child Born out of Wedlock:
- Birth Certificate
- Social Security Card
- One of the following:
- Voluntary Acknowledgement of Paternity (VAP) from the state in which the child was born
- Court Order establishing paternity
Step-child(ren):
- Marriage Certificate
- Birth Certificate must contain the name of at least one (1) parent

Adopted Child(ren):
- Final Adoption Decree or documentation from authorized placement of agency under the Department of Human Services

Ward:
- Court Order granting custody to the Sponsor. Court Order must stipulate that the child will be in the care and custody of the sponsor for no less than twelve (12) consecutive months. The court order can designate length of custody by age, time or permanency.

Retirees (Initial Retirement):
- Initial Retirement: DD 214 (Copy 4)
- Reserve/National Guard Retirees need Orders/Letter transferring to Retired List and authorizing pay
- If Medicare eligible, must present Medicare Card
- Must have two (2) valid, unexpired forms of identification
100% Service-Connected Disabled Veteran (DAV):
- Letter from Department of Veterans Affairs showing member is rated as 100% service-connected disabled with effective date.
- DD 214 (Copy 4)
- Must have two (2) valid, unexpired forms of identification
- Spouse and children will need to provide documentation under Family Member information
Full-Time Students (21-23 years old):
- Letter from Registrar’s Office declaring full-time student with anticipated graduation date. If graduation date is not provided, a letter will be required every semester to maintain benefits.
- Sponsor must be providing over 50% support – must be stated in block 89 (Remarks) of DD 1172-2
- Student must have two (2) valid, unexpired forms of identification

Does my Sponsor have to be with me to get an ID Card?
Yes, with the following exceptions:
- If the sponsor signs DD Form 1172-2: Application for Uniformed Services Identification Card/DEERS Enrollment in front of a Verifying Official, the family member may bring this form unaccompanied by the sponsor.
- If the sponsor signs DD Form 1172-2: Application for Uniformed Services Identification Card/DEERS Enrollment and has it notarized, the family member may bring this form unaccompanied by the sponsor.
- The family member may sign the DD Form 1172-2: Application for Uniformed Services Identification Card/DEERS Enrollment using a current (original) Power of Attorney (P.O.A.).
- The sponsor, who is CAC authorized, may digitally sign the DD Form 1172-2 at the RAPIDS Self Service page: https://www.dmdc.osd.mil/self_service
What documentation is needed to add, remove or make changes to DEERS?
- The sponsor will always need original or certified copies of the documentation to make any changes (i.e., Birth certificate, Marriage Certificate, Divorce Decree, Death Certificate, Social Security card.)
Once the DD Form 1172-2 is signed, how long is it good for?
- 90-days
Can an ID card be issued by mail?
- Yes only for dependents, the Sponsor can log into https://idco-pki.dmdc.osd.mil/idco/ and click on “Family Member” to have a dependent ID card mailed to your residence.
What is the earliest an ID card can be renewed before expiration?
90-days with the following exceptions:
- Card is damaged
- Promotion of Sponsor
- Significant change in appearance
If the sponsor is deployed, how can a family member receive an ID card?
- The Sponsor can log into https://idco-pki.dmdc.osd.mil/idco/ and click on “Family Member” to have a dependent ID card mailed to your residence.

What is the minimum age a dependent can receive an ID Card?
Age 10, with the following exceptions:
- Dependent, dual active service members
- Sponsor is a single parent
- Dependent is not residing in household
How long may a dependent keep an ID card?
- Up to age 21. At age 21, if the dependent is enrolled full-time student in an accredited institution of higher learning, that dependent may have an ID card until the date of graduation or their 23rd birthday, whichever occurs first.
- EXEPTION: The dependent is incapacitated either before age 21, or before the age of 23 if a full-time student. Please visit the DFAS website for information on Incapacitated Children.
What is needed to establish a dependent as a full-time student in DEERS?
- A letter from the school’s registrar’s office stating the dependent is a full-time student AND anticipated graduation date. The sponsor must also be providing over 50% support for the dependent.
Can a parent, step-parent, or parent-in-law be added as a dependent in DEERS?
- Yes. Please visit the DFAS website to apply for Secondary Dependency Determination. Once approved the Service Member will need to bring the forms and newly established dependent to the DEERS/ID card office to be enrolled into DEERS and receive an ID card.
How are family members and wards added to DEERS?

Can an Un-remarried Former Spouse (URFS) of a sponsor keep their ID card and benefits?
Yes, when the following conditions are met:
- The sponsor performed at least 20 years of service, the spouse was married to the sponsor at least 20 years, and the 20 years of marriage were during the 20 years of the sponsor's service then the URFS is entitled to full benefits so long as they do not remarry.
- Or, the sponsor performed at least 20 years of service, the spouse was married to the sponsor at least 20 years, and the 20 years of marriage were during at least 15 years of the sponsor's service, then the URFS is only entitled to medical coverage for 1 year.
- The following documentation needs to be submitted to the DEERS ID Card Section to have record updated in DEERS before issuance of an ID card (this can take up to 60-90 working days):
- Marriage Certificate
- Driver’s License
- Final Divorce Decree (complete)
- DD Form 214 (copy 4)
- Military ID Card (DD Form 1173)
- Photo ID issued by state or local government
- Social Security Card
- Statement from former spouse stating that he/she does not have employer sponsored health insurance and has not remarried. Needs to be dated and signed.
What is required for a Disabled American Veteran (DAV) to receive an ID card?
To receive a DAV ID card, the person must bring:
- The letter from the Office of Veteran Affairs (VA) stating that they are 100% disabled and has entitlements to the Commissary and Post Exchange.
- The Service Members DD Form 214: Certificate of Release or Discharge from Active Duty
- Picture ID and supporting documentation for any and all family members to be added.
- Note: A Disabled American Veterans ID card entitles them to Commissary and Post Exchange privileges only. Medical is handled by the VA.
