Voluntary and Involuntary Separation
The following Separations require the Soldier to have an escort of equal or higher Grade:
- Chapter 5-13 (Personality Disorder)
- Chapter 7 (Defective Enlistment and Extensions)
- Chapter 9 (Alcohol or other Abuse Rehab Failure)
- Chapter 10 (Discharge in Lieu of Trial by Court Martial)
- Chapter 11 (Entry Level Performance and Conduct)
- Chapter 14 (Misconduct)
Request for separation orders should be submitted via PAR to 000000000003410
Voluntary and Involuntary Separation packets required the following Documents:
Memorandum from COL / CG (as applicable) who is the separation authority with paragraph statement “I DIRECT”
Type and “Characterization” of separation must be noted / selected
Þ CDRs Memo: company command MFR with earned awards and ASVAB scores
DD Form 2648 Active-Duty Pre-Separation Checklist from TAP (e-signed form DD 2648)
Personnel Action Request (PAR)- if AWOL, Confinement, Lost time bringing Soldier back to duty PDY (as applicable)
Absence Request signed by at a minimum Company Commander. Leave is at Commanders Discretion.
NOTE: A Discharge of Other Than Honorable (OTH) is NOT authorized leave Other Than Honorable (OTH) is an automatic reduction to E1/PVT
Article 15 documents (as applicable)
Command sponsorship paperwork / ERD (if applicable)
