Some helpful information to start:

  • Once you open the enrollment or renewal specific to your situation, an appointment link will be provided.
  • The ID Card Appointment Scheduler is an application provided by the Defense Military Data Center (DMDC). The DMDC hosts and maintains this site. The Fort Benning ID Card Facility does not control its availability or speed.
  • ID Cards may NOT be renewed more than 90 days prior to expiration.
  • You must make an appointment for each person that will need an ID card.
  • Sponsor must be present for all transactions or Family Member must have current verified DD Form 1172-2 (Application For ID Cards/DEERS Enrollment) or valid General or Special Power of Attorney. Special Power of Attorney must state ID Cards/DEERS enrollments.
  • All customers must have two valid forms of ID on hand (not including their current CAC).
  • A common prerequisite for the issuance or renewal of ANY identification card to adults 18 years or older is the requirement to provide two forms of original identification.  One identity document MUST be an unexpired government issued photo ID from the list of PRIMARY identity documents.  The second identity document may be from the primary list or the secondary list.    The identity source documents must be in the applicant’s name and cannot be expired or cancelled.  If the two identity source documents bear different names, evidence of a formal name change must be provided.  An expired Uniformed Services Identification (USID) Card may be used as a secondary identity source document for re-issuance of a new USID card but is not acceptable for initial issuance.   A link is provided within each specific scenario that describes what constitutes acceptable identity source documents.

 Initial Card Issue

Enroll a Spouse

Documents:

  1. Marriage certificate with license or state filing number (original or certified copy)

Disenrolling a Former Spouse

Sponsor must be present with a certified copy of all pages of the divorce decree.

Enroll a Foreign Spouse with a Foreign Marriage Certificate

  3. Primary Identity Source Document (ACCEPTABLE IDENTITY DOCUMENTS)

Enroll a Child

Documents:

  1. Birth certificate (original)
  2. Social Security Card (original)
** Please be advised, Verification of Birth Facts or Hospital Proof of Birth can no longer be used in lieu of a Certified Birth Certificate.
** Please note, if male sponsor is NOT MARRIED to the mother of the child, please provide a court order, and / or an Approved Dependency Packet from DFAS, or an Acknowledgement of Paternity.

** Contact the Local Vital Records Office: If the acknowledgment was filed in the U.S., reach out to the vital records office in the state where it was originally documented. They can guide you on how to request a certified copy, even from abroad.

** Coordinate with the U.S. Embassy or Consulate: The embassy or consulate in the country where the mother and child are located can assist with notarizing documents, facilitating communication with U.S. agencies, or providing guidance on local procedures.

 **Legal Assistance: If the acknowledgment was made overseas, consult the legal office on the military base or a local attorney familiar with international family law.

 

Enroll a Stepchild

Enroll an Adoptive Child with Adoption Decree

College Student ID Card Eligibility (Ages 21-23)

College students remain eligible for a Family Member ID card between the ages of 21 and 23 as long as they remain unmarried, a full-time college student, and the sponsor provides over 50% of the student's support.  All ID card benefits for college students end at age 23 – no exceptions.  Continued medical benefits after the age of 23 is through the TRICARE Young Adult Program.  More information can be found on the TRICARE website.

Initial Military Retiree ID Card

Spouses

Documents required for renewing or replacing an expiring, lost or stolen card assumes that the individual is already enrolled in DEERS.

Documents Required:

2. Two (2) valid unexpired forms of identification (original) from the DoD List of Acceptable Identity Documents, one of which must be a federal or state issued photo ID.

3.  Family Members approaching age 65 must enroll in MEDICARE PART A and B, and be in possession of the MEDICARE card to update DEERS and to receive a new ID Card.  See information below "ID Card Information for Individuals Approaching Age 65" and "ID Card Information on MEDICARE and TRICARE for Life".

Child (Including Step-Child, Adopted Child and Ward)

Documents required for renewing or replacing an expiring, lost or stolen card assumes that the individual is already enrolled in DEERS.

Documents:

All customers must have two valid forms of ID on hand (not including their current CAC)

 

Children ages 10 to 17 need only one (1) valid unexpired form of identification from the DoD List of Acceptable Identity Documents.

College Student ID Card Eligibility (Age 21-23)

2. Two (2) valid unexpired forms of identification (original) from the DoD List of Acceptable Identity Documents, one of which must be a federal or state issued photo ID.

3.  Retirees who were temporarily retired as a result of a physical disability (Temporary Disability Retired List - TDRL) and are later permanently retired (Permanent Disability Retired List - PDRL) must provide orders indicating the permanent retirement.

4.  Military Retirees approaching age 65 must enroll in MEDICARE PART A and B, and be in possession of the MEDICARE card to update DEERS and receive a new ID Card.  See Information below "ID Card Information for Individuals Approaching Age 65" and "ID Card Information on MEDICARE and TRICARE for Life".

100% Disabled Veteran

Documents:

2. UPDATED VA Commissary Letter (located at https://eauth.va.gov/accessva/)

Parent or Parent-in-Law

Documents required for renewing or replacing an expiring, lost or stolen card assumes that the individual is already enrolled in DEERS.

Documents:

1.  All customers must have two valid forms of ID on hand (not including their current CAC)

2. Parents and Parents-in-law are only approved for a period of 4 years or less.  Each subsequent ID card will require a new Approved Secondary Dependency Packet from Defense Finance and Accounting Service (DFAS).

3.  Two (2) valid unexpired forms of identification (original) from the DoD List of Acceptable Identity Documents, one of which must be a federal or state issued photo ID.

Note: If the sponsor has a change in status, (as an example, retirement or transition to National Guard or Reserve) the renewal process for the parent or parent in law must be reinitiated.

ID Card Information for Individuals Approaching Age 65

Understanding MEDICARE and TRICARE for Life

If the information below pertains to you please go to either the "Spouse" or "Military Retiree" section above to review the documents needed and to make an appointment.

The window for enrollment for MEDICARE spans seven months (3 months prior to birth month, birth month at age 65, and 3 months following). If you do not enroll at age 65, there is a penalty, and could limit access to Tricare Prime or Tricare select coverage until you eventually enroll in MEDICARE.  Although the MEDICARE window spans several months, BE ADVISED, if you are approaching age 65, check the expiration date of your ID card; it should expire on the last day of the month preceding your birth month. At that time your medical benefits under regular TRICARE will also expire. Enrollment in MEDICARE PART A and B, and possession of your MEDICARE card is required for update in DEERS and issuance of a new ID card which enrolls you in TRICARE FOR LIFE. It is best to enroll in MEDICARE at the earliest possible date to ensure seamless transition of medical benefits. To enroll in MEDICARE parts A and B, contact the Social Security Administration at (800) 772-1213 (online at www.ssa.gov) or MEDICARE at 800-633-4227 (online at www.medicare.gov). If you are deaf or hard of hearing, you may call the Social Security TTY number (800) 325-0778. You can also visit your local Social Security office. If you apply online, once approved, you can print your own MEDICARE card. If you are enrolled in MEDICARE when you are first eligible, but do not yet receive Social Security, MEDICARE will bill you for your MEDICARE Part B premiums. MEDICARE Enrollment and TRICARE for Life TRICARE for Life (TFL) is available to TRICARE retirees, family members and beneficiaries, regardless of age or where you live. If you have Medicare Part A and Medicare Part B, you’re eligible for TFL on the first date you have both Medicare Part A and Medicare Part B. Medicare Part A covers inpatient hospital care, hospice care, inpatient skilled nursing facility care, and some home health care.

The Social Security Administration (SSA) determines your entitlement to Medicare Part A based on your work history or your spouse’s (this includes divorced or deceased spouses) work history.

Medicare Part B covers provider services, outpatient care, home health care, durable medical equipment, and some preventive services. Medicare Part B has a monthly premium, which may change yearly and varies based on income.

BE ADVISED, if you are approaching age 65, check the expiration date of your ID card; it should expire on the last day of the month preceding your birth month.

20/20/20 OR 20/20/15 Former Spouse (FS)

Documents Required

1.      All customers must have two valid forms of ID on hand (not including their current CAC)

5.      SSN card

6.      If on Active Duty -Statement of Service from Soldier Command.

7.      Reservist:  FS of USAR must include a final or current 5016 (point of statement).

8.      National Guard: FS of National Guard sponsors must include a final or current NGB 23.

9.      Statement from former spouse that he/she does not have employer sponsor health insurance and has not remarried.

10.      Two Forms of Photo Identity:  Driver License, Passport or State ID
