Some helpful information to start:

  • Once you open the block specific to your type of card an appointment link will be provided.

  • Common Access Cards may NOT be renewed more than 90 days prior to expiration.

  • The ID Card Appointment Scheduler is an application provided by the Defense Military Data Center (DMDC). The DMDC hosts and maintains this site. The Fort Benning ID Card Facility does not control its availability or speed.

  • A common prerequisite for the issuance or renewal of ANY identification card to adults 18 years or older is the requirement to provide two forms of original identification.  One identity document MUST be an unexpired government issued photo ID from the list of PRIMARY identity documents.  The second identity document may be from the primary list or the secondary list.    The identity source documents must be in the applicant’s name and cannot be expired or cancelled.  

  • If the two identity source documents bear different names, evidence of a formal name change must be provided.  An expired Common Access Card (CAC) may be used as a secondary identity source document for re-issuance of a new CAC but it is not acceptable for initial issuance.  A link is provided within each specific scenario that describes what constitutes acceptable identity source documents.   

  • LOST OR STOLEN USID & CAC ONLY REQUIRE A POLICE OR MP REPORT
    IF ID CARD IS CONFISCATED, PLEASE BRING IN THE CONFISCATION MEMO TO YOUR APPOINTMENT!

DA Civilian -Initial Issue - Renewal - Lost - Stolen

Initial Issue

All data must be previously input through the Civilian Personnel Advisory Center (CPAC).

Must have two (2) valid unexpired forms of identification from the in original form from the DoD List of Acceptable Identity Documents.

Renewal

Must have two (2) valid unexpired forms of identification in the original form from the DoD List of Acceptable Identity Documents.

Lost / Stolen

Must have two (2) valid unexpired forms of identification in the original form from the DoD List of Acceptable Identity Documents.

Lost or Stolen CAC / ID Cards require a Police or MP Report.

Lost or Stolen CAC / ID Cards require a memo from CPAC Office Bldg 4

Contractors - Initial Issue - Renewal - Lost - Stolen

Initial Issue

All data must be previously input through Trusted Associate Sponsorship System (TASS).

If going overseas, a Letter of Authorization (LOA) must be presented reflecting this information

Must have two (2) valid unexpired forms of identification in the original form from the DoD List of Acceptable Identity Documents.

Renewal

Must have two (2) valid unexpired forms of identification in the original form from the DoD List of Acceptable Identity Documents.

Lost / Stolen

Must have two (2) valid unexpired forms of identification in the original form from the DoD List of Acceptable Identity Documents.

Lost or Stolen CAC / ID Cards require a Police or MP Report.

Lost or Stolen CAC/ID Cards require a memo from MP ICAM/Employer

Soldiers - Renewal - Lost - Stolen

Most Soldiers at Fort Moore are assigned to units that have the capability to produce Common Access Cards (CAC).  Soldiers assigned to those units must receive CAC support from their Brigade S1.

Soldiers with Lost or Stolen CAC / ID Cards must provide a Developmental Counseling , DA Form 4856 digitally signed by an Officer or Senior NCO in the grade of Sergeant First Class or above in the Soldier’s chain of command

In an emergency, if the system is not operational, Brigade S1s should coordinate with other Fort Moore ID Card sites and the Fort Moore ID card section for support.

Soldiers assigned to units that ARE supported by the Fort Moore ID Card Facility must make an appointment.

Must have two (2) valid unexpired forms of identification in the original form from the DoD List of Acceptable Identity Documents.

Lost or Stolen CAC / ID Cards require a Police or MP Report.

(Military males must have clean shaved face unless you have a profile or religious exception to receive CAC.

(Not require for retiree or dependents cards).
