Due to the vaccine administration requirements changes have been made to how, when and where the COVID-19 shots will be given regardless of appointment status. Be Advised we require 10 individuals to sign up in order to administer any doses. If we do not have enough people sign up, your appointment may be cancelled. The location is the health clinic at this time not the Howard Gym. If you have any questions about the vaccine, navigating the appointment site, or your appointment, please call 928-328-2666. If you need to cancel please call ahead of your scheduled appointment time.

- Per new Defense Health Agency guidance regarding social distancing and COVID-19, routine care such as well child and adult physicals will now be deferred to a later date.
