BLC PACKET REQUIREMENTS
MLC PACKET REQUIREMENTS
1. TRADOC form 350-18-2-R-E (Pre-Execution Checklist)
2. Permanent/Temporary Profile (if applicable)
3. Most recent ACFT Scorecard- DA 705
4. Supplemental Body fat Contact worksheet (DA 5500/5501, JUNE 2023) if applicable. No older than 30 days.
5. If over 40, bring a completed cardiovascular screening as part of the PHA,
within the past 12 months, to attend NCOES IAW AR 40-501, Chapter 3-14a (5).
6. Updated SRB within last 30 days.
7. A notebook/notepad and a pen.
8. Copy of ATRRS enrollment email and/or Orders (2 copies of 1610 if TDY).
9. MLC Student Data Sheet.
