This facility services Dependents, Retirees, Civilians, Contractors, and other Nonmilitary Members. We also take care of enrollments, Pin Resets, DEERS updates (divorce decrees, address change, etc.) and DD 1172-2.

If you are a Tenant Unit not assigned to a Brigade, National Guard, Reserve, ROTC student, or another branch of service not Army in need of CAC services, you will go to the Mower Processing Center at 614 Harmon Avenue, BLDG 208 located across from the Soldier Service Center. All Soldiers assigned to units with CAC/ID Card capabilities must be SERVICED by their Brigade S1or their Sister Brigade if their S1 is down for CAC services.

 

Two Valid, unexpired forms of identification must be present for dependents 18yrs and older. One must be a valid, state or federal photo ID. All documents must be ORIGINALS or CERTIFIED TRUE COPIES. Local Badges and License to Carry Permits are not accepted. See list of acceptable documents. CURRENT AS OF 14 MAY 2025: “Definition of valid forms of identification mean unexpired forms of  Identification that are NOT a CAC, that a customer should have on file (e.g. Drivers License, Passport, Birth Certificate, Social Security Card, etc.).” –DMDC Project Office

 

*NOTE* If a record is not compliant with having two forms of identification (non-CAC identification) on file, RAPIDS will NOT allow reissuance.

EXCEPTIONS Dependent must present a valid POA for ID Cards/DEERS. A DD FORM 1172-2 signed by the sponsor from an ID Card Office or online at https://idco.dmdc.osd.mil/idco/ within 90 days is acceptable for ID card renewals only. This information pertains to all dependents regardless of the situation.

All documents MUST be original, court certified copies, or certified true copies. Documents in any other form will NOT be accepted. NAMES NEED TO MATCH on documents, (Driver's license, Passport, SS Card). If you have foreign documents issued outside of the USA, please refer to the 'Foreign Documents' section.

Foreign documents NOT in English must be translated through Military One Source at 1-800-342-9647 (for Active Members), or from a certified translator. All foreign documents must have an Apostille or Legalization from the Embassy signed by a U.S. Consular. Bring in the original document from the country of origin and the certified translation along with the Apostille or Legalization.

MP report/Police Report (within 90 days of reported incident, otherwise a new report is needed). Two valid forms of ID (check list of acceptable documents).

What to bring when adding a spouse:

Marriage Certificate recorded with the state of issuance, Social Security Card (letter from SS Office called a Snap Form with SS number), Valid photo ID such as driver's license. (check list of acceptable documents). If applicable, Divorce Decree, Annulment, or Death Certificate of previous spouses (sponsor and spouse), Medicare Card. (Names must match on both forms of ID)

Adding a child:

Birth Certificate (Must list at least one parent), SS Card. Male sponsors adding children born out of wedlock need voluntary paternity of acknowledgment or court order paternity and must be present for the enrollment. (Names must match on both forms of ID) EFFECTIVE 13 APR 2022, we are no longer able to accept the Hospital Proof/Confirmation of Birth.

 

 Adding a stepchild:

Birth Certificate (Must list at least one parent), SS Card, Marriage Certificate. If child is born to male spouse, will also need voluntary paternity of acknowledgment or court order paternity. 

Adding a child due to adoption:

Final Adoption Decree, New Birth Certificate (Must list at least one parent), New SS Card

 

For Full-time Students

Two valid forms of ID (check list of acceptable documents). MP report/Police Report (within 90 days of reported incident, otherwise a new report is needed). Full-time students (21-22) also need new verification letters from college.

If the spouse cannot come with the dependent, they can come to DEERS and complete DD 1172-2, or the dependent can have a valid power of attorney.

 

Birth Certificate (Must list at least one parent), SS Card, Court order granting custody to the sponsor. The sponsor must provide over 50 percent support. Child must reside with sponsor for a year or longer.

Birth Certificate (Must list at least one parent), SS Card, Approved Memo from DFAS for secondary dependency. The sponsor must provide over 50 percent support. Child must reside with sponsor for a year or longer.

Must be attending an accredited institution. The sponsor must provide over 50 percent support. Two forms of valid ID (check list of acceptable documents). Child must not be married. ID can be renewed within 90 days of turning 21

Letter from the school registrar signed certifying current full-time status for associate's degree or higher and anticipated graduation date along with current term dates. The letter must be presented to DEERS by the sponsor. College transcripts or class schedules are NOT acceptable Students on Break must provide enrollment verifications for the semester before and the semester after break to include grad date, term dates, degree, and full time for each semester. 

Unmarried Tricare Young Adult (21-26)

Two valid forms of ID (check list of acceptable documents). Old ID card. ID CANNOT be issued before the effective date. Call Tricare to add a child to Tricare Young Adult. Sponsor does need to be present unless the sponsor comes into DEERS within 90 days and completes the DD1172-2 or provides the child with a Power of Attorney to include DEERS/ID card functions.

SS card, Old ID card, medical sufficiency letter signed by physician (check the DFAS website below for a sample of requirements) Approved Memo from DFAS for secondary dependency. Valid photo ID (check list of acceptable documents) SS card, Birth certificate, Birth Certificate of spouse or sponsor related to the parent, Approved Memo from DFAS for secondary dependency completed through DFAS.

DoD Civilian Retiree ID Cards will no longer be issued. Previously issued DoD Civilian Retiree ID Cards will remain valid through 31 August 2023 and will not be reissued, PER the Department of Defense Manual (DODM) 5200.08 Volume 3. Contact the Visitor Control Center for further guidance concerning future installation visits.

Letter from VA showing member rated 100 percent disabled and authorizing Commissary, Post Exchange privileges, and MWR and VA papers showing start date of 100 percent disability or the summary of benefits showing total and permanent disability. DD FORM 214 (copy 4), must show that you are honorable or under honorable conditions. Two valid forms of ID (check list of acceptable documents). NOTE: If you are military retired or a family member with a dependent ID card, you will not be able to receive the 100 percent DAV card as your ID Card already provides Commissary, PX, MWR access.

 

Two valid forms of ID (check list of acceptable documents). All dates must be previously input through DCPDS and can take up to 24 hours to update in DEERS. Check with your HR personnel. If you go overseas, you must present the DD FORM 2365 and DD FORM 1610.

Appointments for Retirement ID Cards should not be made until the day of the Retirement (after the separation date) or later. Retirement Orders, DD FORM 214 (copy 4), Two valid forms of ID (check list of acceptable documents), Old ID Card. If Medicare is eligible, bring in a Medicare card.

Retirement Orders issued at age 60, Old ID Card, Two valid forms of ID (check list of acceptable documents). ID cards cannot be issued until the 60th birthday or later.

Old ID card, two valid forms of ID (check list of acceptable documents), Medicare card showing part A and B. Please apply for Medicare at least 90 days before your 65th birthday so that Medicare can be reflected in DEERS.

Orders moving sponsor from TDRL to PDRL. Two valid forms of ID (check list of acceptable documents). Old ID card.

An appointment should be made the day after the separation date or later. Bring DD Form 214 (member 4), Separation Orders, Two valid forms of ID (check list of acceptable documents), and an Old ID card. (Must be qualified for TAMP).
