Sponsorship

To initiate the Fort Benning Sponsorship Program it is essential that Soldiers complete the DA Form 5434 (Sponsorship Program Counseling and Information Sheet) in Army Career Tracker (ACT) TASP under Sponsorship during their reassignment processing at their last duty station. All Soldiers (E1 - O6) must login and complete the sponsorship request form in ACT. Click here for the link to ACT TASP. You can use your CAC or register to create a DS Logon User ID and password in ACT. The DA Form 5434 advises the command of any special needs and support mechanisms that the Total Army Sponsorship Program must muster to help the Soldier and his family. The completed DA Form 5434 will be automatically sent to the gaining installation for further processing. All incoming military personnel are required to sign-in at Installation Replacement Operations, Building 35 - Ridgeway Hall, 35 Ridgway Loop. After duty hours and weekends/holidays, personnel will sign in at McGinnis Wickham Hall with the Staff Duty NCO. If you have any questions, please call In-processing 706-545-4577

To request a sponsor: Complete the online ACT TASP form, DA 5434, at the above link, save and it will automatically be forwarded to the Soldier projected gaining installation for assignment of sponsor.

If you are unable to open the ACT TASP DA5434 contact your current brigade/battalion ACT TASP Coordinators for assistance.
