Can we choose any school?

The Vernon Parish School Board (VPSB) the installation’s local education agency (LEA) does allow out of zone attendance. Families must complete an out-of-zone attendance agreement.

How can the Military Interstate Child’s Compact Commission (MIC3) help our Family during the transition?

The MIC3 supports Families with academic placement,Individual Education Plans (IEPs) and Individual Accommodation Plans (IAPs/504Plans).

In addition it provides guidance for best practices relatedto high school graduation and transcript transfers as well as access toextracurricular activities and sports.

What intervention and enrichment activities do the schools have?

Our Local Education Agencies offer Response TieredIntervention (RTI). This provides smallgroup instruction in core academic areas.

The schools also offer various enrichment activities perschool based on student interest, grant funding and school needs assessmentdata.

Extracurricular activities are also offered based on studentinterest and availability of school resources.

Where can we get notary services?

TheFort Polk Office of Soldier and Family Services provides free notary servicesfor Soldiers, Dependents, and Retirees on a walk-in basis. If you need notary services, come by the Officeat Building 1454 (next to the Showboat) Monday – Friday 0830 – 1130 and Monday– Thursday 1330 – 1600. How do we sign up for Child Care?

Goonto MCC.com (Military Child Care), create an account, search and requestcare, manage your request, and ensure that your profile is up to date. Wealso recommend that you come to building 400 parent central services toregister your child prior to being offered a care.

How do I sign my child up for Sports?

CallParent and Outreach services at 337-531-1955 to set up an appointment for aninitial registration at building 400. You will need to have a currentsports physical when you come. Once you have registered you can stop in atbuilding 400 and sign your child up for sports and camps. **Sports Physicalsare only good for one year**

How much is Child Care?

Careis based off your total family income. During registration for care we will askthat you bring the last two current LES/ paystubs for both sponsor and spouse,we can then help you determine how much you would pay for care.

Want to start a home-based business?

Home Based Business, or HBBs, are operated by family members of the U.S. Army soldiers or civilians who provide a service to other U.S. forces members and affiliates. Family members stationed at Fort Polk may want to work out of their home and try their hand at starting a business in or out of their government quarters.

Examples of services are tutoring, singing and dancing lessons, accounting and tax services, word processing, childcare and similar skills. In addition, multi-level sales activities (i.e. Avon, Tupperware and Pampered Chef) are typical HBB activities.

How do I transfer my child’s IEP (Individual Education Plan)?

Discuss MIC3 (military interstate compact agreement) and walk themthrough the process of IEP cover-letter. Provide school specific informationbased off grade(s) and Special Education department for Vernon or BeauregardParish. Offer to review IEP goals/ accommodations and discuss transitions; completeFamily Needs Assessment/ Family Service Plan.

How do I transfer my current referrals/ make an appointment with a provider for my EFM and Family?

Service dependent. Discussmedical records, needs to transfer, dependent on provider location (on/offbase). Provide form and instructions for transferring medical records fromprevious PCM or specialists. Discuss Case Management roles and provide information/referral, case dependent.

I have a family member who I think needs to enroll; what do I do? (Often with move, unfamiliar of EFMP until newcomers, or new diagnosis)

Walk through EFMP; typicallycompleting Family Needs Assessment/ Family Service Plan. Provide enrollmentforms, instructions, welcome letter, flyer, and resources via email, or packetif in person. Discuss family needs and if services are needed through FNA/ FSP.

My Family member need accommodations in housing, can you assist?

Discuss needs and coordinatewith Corvias. Verify enrollment in EFMP prior to referral. Assist inaccommodations where needed; complete Family Needs Assessment.

How do I register my child in the EFMP program for childcare?

Provide information on CYS andforms. Discuss Multidisciplinary Inclusion Action Team (MIAT) to provideaccommodations for EFM in the CYS setting and provide MIAT flyer.

I have a family member enrolled, how and when do I need to update enrollment?

Verify validation date. Provide EFMP enrollment forms and instructions.Discuss family needs and if services are needed through FNA/FSP.

I am active duty and need urgent care. What are my options?

Seek care at a military hospital or clinic when/whereavailable or contact your PCM or call the Nurse Advice Line 1-800-TRICARE(874-2273) to request a referral before getting civilian care.

I am a dependent of an active duty and need urgent care. What are my options?

: If enrolled to TRICARE Prime no referral is requiredbut you must use TRICARE authorized-network providers to avoid out of pocketcost.

I am TRICARE Select and need urgent care; what are my options?

You don’t need a referral. Youcan get urgent care from any TRICARE authorized urgent care center or provider.

How much does it cost to enroll dependents of active duty into dental?

One dependent is: $11.60 monthly; 2 or more dependents- $30.15 monthlysubject to change each year.

I am getting married and want to add my spouse and step-child to TRICARE. What do I need to do?

Your spouse and step-child(ren) must be registered in DefenseEnrollment Eligibility Reporting System (DEERS). 1-800-538-9552 or http://milconnect.dmdc.osd.mil and TRICARE within 90days.
