In accordance with 32 CFR Part 161 Subpart D 161.23 {b}(3), the Hospital Proof of Birth Letter is NOT an acceptable document for enrolling newborns. The ONLY acceptable documents are:

  • Birth Certificate
  • Certificate of Live Birth
  • Consular Report of Birth Abroad (FS-240)

A definition for a certificate of live birth is a certificate authenticated by an attending physician or other responsible person from a U.S. hospital or a military treatment facility showing the name of at least one parent.

If questions arise concerning legal requirements of a Certificate of Live Birth, those questions should be directed to the hospital's supporting legal office.​

The ID Cards/DEERS section is an appointment based system. We accept walk-ins on a first come, first served basis as manpower allows. Appointments can be scheduled 30 days prior to card expiration. Cards can be renewed 90 days prior to expiration. you can schedule an appointment by visiting the rapids id card office online.

Sign-in for walk-ins are accepted from 7:30 - 11 a.m. and from  1 - 3 p.m. Time subject to change due to increased volume and may be restricted by priority. Note: Lost CACs are required to provide a MP report to have CAC replaced.
