WHO WE SERVE: 

The Fort Moore ID Card Facility provides Common Access Cards (CAC) for Department of the Army (DA) Civilians, Government Contractors and limited Soldier CACs for select units.  In addition, we provide Uniform Services Identification (USID) Cards for Military Retirees and Family Members of Active Duty, Retiree and DAV.

OPTIONS FOR RECEIVING ID CARD SERVICE:

There are two options available for customers to receive ID card services.

1.  Schedule an  appointment

2.  Utilize the remote ID card processing option.

Some helpful information to start:

All customers must have two valid forms of ID on hand (not including their current CAC).

Lost or stolen USID & CAC only require a police or MP report
if id card is confiscated, please bring in the confiscation memo to your appointment!

You must make an appointment for each person that will need an id card. (if spouse is not present please bring DEERS, special power of attorney for authorization to issue id card.)

All sponsors please bring one of the following documents your original document (birth certificate, certificate of naturalization N-550 or N-570; certificate of citizenship N-550 or N570)
