NOTE: If an expired military ID card is presented at the gate as you enter the installation, the gate guard WILL confiscate it. They should give you a memorandum. Please bring the memorandum with you to the ID Card Section.
WARNING Photocopying U.S. Government Identification cards is a violation of "Title 18, US Code Part I, Chapter 33 Section 701 and punishable of both fine and/or imprisonment. Please note: this does not apply to medical establishments (Doctor's office, hospitals, pharmacies) who are allowed to make a copy for insurance claims or medical supplies and other government agencies in the performance of official government business. |

Proofs of Identity are mandatory.

Two (2) valid, unexpired forms of identification must be presented to receive any ID card. For a list of authorized proofs of identity, please click here.

All documents must be originals or they must be certified or notarized copies.

All court documentation must be state or county certified stating that it has been filed and recorded in the courthouse.

When a Social Security (SS) number is required, you may use a SS Card or other official documentation showing the SS number (i.e. Tax ID form, letter from SS Office with SS number or Military ID).

Driver’s Licenses must have an actual date of expiration.

Foreign documents must be translated by an official government translator.

If going overseas, a Letter of Authorization (LOA) must be presented reflecting this information

NOTE: If student falls below full-time status, the ID card must be turned in to a local ID card facility. Any medical costs incurred will be the responsibility of the Sponsor.
