Request for Military Funeral Honors:
All requests for Military Funeral Honors, please provide a copy of the DD Form 214, Certificate of Release or Discharge from Active Duty or an official honorable discharge certificate of the deceased to the Casualty Assistance Office as soon as possible. All funeral requests should be received 48 hours prior to the Military Funeral Honors being rendered.
